EFEKTIVITAS EKSTRAK KULIT MANGGIS(Garcinia mangostana) TERHADAP JUMLAH LIMFOSIT T CD4+ DAN KADAR IL-2 PADA PENDERITA HIV DENGAN TERAPI ANTIRETROVIRAL

Abstract

Background : HIV attacks human’s CD4+ T lymphocytes immunity system. IL-2 initiate cell cycle progression and CD4 T Lymphocytes proliferation. IL-2 becoming Indicator of immunity system repair within HIV infection2. HIV patients under ARV medication with high virus level and high ROS (Reactive oxygen species). ROS can induce apoptosis on CD4+ and raise inflammation. Mangosteen Peel Extract (MPE) contains high antioxidant level which can suppress ROS using its active ingredients, xanthone. This research purposed to prove effectiveness MPE to raise CD4 T ymphocytes and IL-2 on HIV patients under ARV therapy. Method : The research’s method is an experimental with double blind randomized control group pretest-posttest design. Research subjects are patients within ARV therapy > 6 months, selected by inclusion and exclusion criteria. Patients divided into 2 groups which is control (placebo) and treatment (given MPE) dosed 2400 mg/day, consumed in 30 days. Tests was done before and after the treatment to analyze CD4 T Lymphocytes number and IL-2 level. Result :MPE Group and placebo group shows increasing number on CD4+ T cell lymphocytes resulting P<0,001. Average amount of CD4+ T cell lymphocytes in pretest 373,9±127,1 cell/mm3 and increase in post test with average 401,9±152,2 cell/mm3. Il-2 level on MPE group decreased with average from 26,7±32,9 to post test 22,1±26,5 on post test, an insignificant difference with P value 0,260 (p>0,05).But, There were no significant decrease in the level of IL-2 from baseline in the treatment group (p=0.260) and there were significant decreasein placebo group (p=0,008).Spearmans correlation shows r=-0,121 for MPE group and r=-0,018 for the placebo group. Negative values shows opposite direction. Significant correlation between CD4+ T cell lymphocytes and IL-2 level. Conclussion : Mangosteen (Garcinia mangostana) peel extract act as antioxidant that supress apoptosis that will keep the level of IL-2 and the number of CD4+ T cells constant.
