Abstract
ABSTRACT Background: Vitamin E is the principal lipid soluble antioxidant, also known to inhibit platelet aggregation and adhesion, and regulate immune response. This study was aimed to investigated the influence of vitamin E suplementation on the improvement of neurological status in acute ischemic stroke . Methods: A double blind, pre test-post test randomized control trial study in July 2005-July 2006. Subjects were 92 in patients acute ischemic stroke, divided in those who accept stroke ischemic standard therapy + placebo(n=46) and those who accept stroke ischemic standard therapy + Vitamin E 400 IU daily (n=46) for 4 weeks. The severity of neurological deficit was measured by Orgogozo Scale administered on admission, day 7, 14, 21 and 28. The alteration of score was analyzed within and among groups. Data analyzed using 2, Mann-Whitney, Wilcoxon, Friedman test, and General Estimating Equation. Results : After day 28, the mean Orgogozo score increased significantly in both of groups, with vitamin group have higher score. The mean Orgogozo scores in placebo group vs vitamin group were : day-7th 60,2 (15,38) vs 67,3 (17,75); day- 14th 61,8 (16,71) vs 69,9 (18,03); day-21th 64,5 (17,80) vs 74,1 (18,72); day-28th 65,3 (17,96) vs 77,3 (17,75). This among groups difference was statistically significant (p<0.05). Delta Orgogozo score in placebo group vs vitamin group was 5,8 (7,40) vs 12,7 (7,65); p<0,05. The value of Relative Risk Reduction 70,6%, Absolute Risk Reduction 26,1 %, Number Needed to Treat 4 patients. Conclusions : Vitamin E 400 IU daily suplementation in standard therapy gives better improvement in status neurological in mild acute ischemic stroke based on Orgogozo Scale.
