ANALISIS BIAYA UNIT RAWAT INAP PUSKESMAS KELING I KABUPATEN JEPARA TAHUN 2004

Abstract

Untuk mendukung pembangunan kesehatan diperlukan sarana pelayanan kesehatan, salah satunya adalah puskesmas yang bertanggung jawab menyelenggarakan pelayanan kesehatan tingkat pertama. Kabupaten Jepara memiliki 20 puskesmas, tujuh diantaranya merupakan puskesmas dengan fasilitas rawat inap. Puskesmas Keling I adlah puskesmas rawat inap dengan jumlah tempat tidur sebanyak 30 tempat tidur, 6.256 jumlah hari rawat dan BOR rata-rata tahuh 2004 adalah 62,2% dengan beberapa masalah diantaranya belum pernah dilakukan penelitian analisis biaya rawat inap puskesmas di Kabupaten Jepara. Penelitian ini bertujuan untuk mengetahui besar biaya satuan (unit cost) rawat inap Puskesmas Keling I pada ruang perawatan kelas I, kelas II dan kelas III serta berapa besar tingkat pemulihan (CRR=Cost Recovery Rate) pad amasing-masing ruang kelas perawatan untuk kemudian dibandingkan dengan Perda Kabupaten Jepara Nomor 15 tahun 2002 tentang retribusi pelayanan kesehatan pada puskesmas Kabupaten Jepara. Metode penelitian ini adalah penelitian studi kasus yang bersifat deskriptif, dengan tujuan mengetahui berapa besarnya biaya satuan unit cost) aktual dan angka tingkat pemulihan biaya (CRR) dengan mempertimbangkan komponen biaya gaji, investasi, biaya obat, biaya umum (ATK dan BHP non kesehatan, listrik, telepon), biaya makan pasien, biaya jasa pelayanan, biaya pemeliharaan dan kebersihan, biaya honor, dengan perhitungan menggunakan metode distribusi ganda (double distribution) Hasil penelitian didapatkan biaya total (total cost) tanpa gaji dan investasi (AFC) untuk kelas I Rp 142.820.406,73 kelas II Rp 128.090.389,97 dan kelas III Rp 584.889.469,49, sedangkan biaya satuan unit cost) aktual tanpa gaji dan investasi (AFC) untuk kelas I sebesar Rp 138.257,90 (CRR 20,97%), kelsa II Rp 139.989,50 (CRR 13,57%), kelas III/umum Rp 135.768,21 (CRR 10,31%), biaya satuan unit cost) aktual gabungan sebesar Rp 138.005,20 (CRR 14,95%), sehingga kegiatan pelayanan rawat inap di Puskesmas Keling I mengalami defisit anggaran dan memerlukan subsidi dari pemerintah. Dalam penelitian hasil perhitungan biaya satuan unit cost) dengan usulan kenaikan tarif berdasarkan hasil survey ATP/WTP peneliti pada bulan Juni Juli 2003 untuk kelas I Rp 125.000,00(CRR 90,91%), kelas II Rp 100.000,00 (CRR 71,43%), dan kelas III Rp 90.000,00 (CRR 66,29%)/biaya riil yang dikeluarkan oleh pasien kelas I Rp 150.235,22 (CRR 108,66%), kelas II Rp 136.498,05(CRR=97,79%) dan kelas III Rp 135.680,66(CRR 99,93%). Dengan kenaikan tarif maka diharapkan beban pemerintah daerah akan lebih ringan, dan akan meningkatkan pelayanan kesehatan yang bernutu dan berkualitas. This research aim to know bigly of expense unit cost in patient departement of Keling I Public Health Centre at room treatment class of I, class of II and class of III and also how big cure storey level (CRR=Cost Recovery Rate) at every treatment class room for then compared to Perda Sub-Province of Jepara Number 15 year 2002 about retribution service of Public Health Centre District at Jepara. This research method is descriptive case study research, with a purpose to know how much actual unit cost and actual Cost Recovery Rate by considering salary expense component, invesment, expense of drug, general expense (office stationary and materials used upwear the non health, electrics, telephone), expense eat patient, expense of service, maintenance cost and higyene, honourable expense, with calculation use double distribution method. The resource's result is total cost without salary and invesment (AFC) for the class I Rp 142.820.406,73 class of II Rp 128.090.389,97 and class of III Rp 584.889.469,49, sedangkan biaya satuan while actual unit cost without salary and investasi (AFC) for the class of I equal to Rp 138.257,90 (CRR 20,97%), class of II Rp 139.989,50 (CRR 13,57%), class of III/common Rp 135.768,21 (CRR 10,31%), biaya satuan aliance actual unit costequal to Rp 138.005,20 (CRR 14,95%), so that activity of in patient department servuice in Keling I Public Health Centre budget deficits and need subsidy of goverment. In research of result count of unit cost with proposal increase of healthy service cost based on result of ATP survey/researcher WTP in June Juliy 2003 for the class of I Rp 125.000,00(CRR 90,91%), class of II Rp 100.000,00 (CRR 71,43%), and class of III Rp 90.000,00 (CRR 66,29%)/real expence which released by first class patient Rp 150.235,22 (CRR 108,66%), class of II Rp 136.498,05(CRR=97,79%) and class of III Rp 135.680,66(CRR 99,93%).
