ANALISIS BIAYA PELAYANAN RAWAT INAP DI PUSKESMAS SOKARAJA I KABUPATEN BANYUMAS TAHUN 2003

Abstract

Pelayanan kesehatan yang bermutu dapat terwujud apabila terpenuhinya berbagai fasilitas yang menjadi standar pelayanan dan meningkatkan baik jumlah maupun kualitas sumber daya manusia yang diperlukan. Pada saat sekarang dimana keadaan biaya pelayanan masih tidak memungkinkan untuk mewujudkan pelayanan kesehatan yang bermutu. Masalah penelitian yang diangkat adalah berapa besar biaya satuan (unit cost) rawat inap Puskesmas dan berapa besar CRR (Cost Recovery Rate) dari dari rata-rata biaya pelayanan yang dibebankan kepada satu orang pasien rawat inap Puskesmas per hari serta berapa besar biaya satuan (unit cost) makan pasien, biaya satuan (unit cost) visit dokter, biaya satuan (unit cost sarana dan berapa besar CRR (Cost Recovery Rate) dari masing-masing tarif pelayanan tersebut. Penelitian ini termasuk penelitian studi kasus (case study) yang bersifat deskriptif, yang bertujuan untuk mengetahui berapa besar biaya satuan (unit cost) aktual rawat inap pasien Puskesmas Sokaraja I Kabupaten Banyumas dan mengetahui tingkat pemulihan atau pemenuhan dari rata-rata biaya perawatan yang dibebankan kepada satu orang pasien rawat inap Puskesmas per hari terhadap biaya satuan (unit cost) aktual (Cost Recovery Rate). Serta mengtahui berapa besar biaya satuan (unit cost) makan pasien, biaya satuan (unit cost) visit dokter, biaya satuan (unit cost) sarana dan berapa besar CRR dari tarif yangt berlaku. Metode perhitungan analisis biaya yang digunakan adalah metode distribusi ganda ( double distribution). hasil penelitian didapat biaya satuan (unit cost)tanpa investasi, gaji PNS dan honor dari Pemerintah adalah sebesar Rp.134.948,32 (CRR 69,15%). Biaya satuan (unit cost) makan pasien sebesar Rp.5.042,33 (CRR 59,50%), biaya satuan (unit cost) visit dokter sebesar Rp.3.721,92 (CRR 73,91%). CRR biaya satuan (unit cost) aktual rawat inap dengan rata-rata biaya yang dibebankan kepada satu orang pasien rawat inap Puskesmas per hari (Rp.93.312,00) sebesar 69,15%. hal tersebut berarti kegiatan rawat inap di Puskesmas Sokaraja I mengalami defisit anggaran serta masih memerlukan subsidi dari Pemerintah daerah sebesar 30,85%. hasil perhitungan biaya satuan (unit cost) rawat inap doi Puskesmas Sokaraja I dan dengn ditemukannya besar CRR dapat ditentukan berapa besar tarif yang diinginkan maupun berapa besar biaya perawatan satu orang pasien per hari yang layak. Penyesuaian tarif atau biaya pelayanan kesehatan sangat diperlukan agar terwujud pelayanan kesehatan yang bermutu. Now the situation of healthy service cost is still low, it is imposible to realize the healthy service which has quality. Topic of resource's problem are how much unit cost of Public Service Center's opname, how much CRR (Cost Recovery Rate) from treatment cost average to one opname's patien in Public Health Center petr day, and how much unit cost of patient eating, unit cost of doctor visiting, unit cost of equipment, and how much CRR from each service's rate. This research is descriptive case study research. Which used to know how much actual unit cost of patient's opneme in Sokaraja I Public Health cender in Banyumas and know restoration and fulfillment from treatment cost average to one opname's patient in Public Health center per day with actual unit unit cost (CRR). And to know how much unit cost of patient eating, unit cost of doctior visiting, unit cost of equipment, and how much CRR from the rate. cost analysis method that use is double distribution method. The resource's result is actual unit cost witout investation, government employed and honorarium from the government is about Rp.134.948,32 (CRR 69,15%). unit cost of patient eating is about Rp.5.042,33 (CRR 59,50%), unit cost of equipment is about Rp.1.352,92 (CRR 73,91%). CRR of actual unit health center per day (Rp.93.312,00) is about 69,15%. It means opname activity in Sokaraja I Public Health Center gets devisit budget and still subsidi from local government about 30,85%. The result of unit cost's opname in Sokaraja I Public Healh Center and CRR founded can decide how much the rate they want and how much a suitable treatment cost for one patient per day sutable rate or healthy service cost is very needed to realized the healthy service which quality.
