Abstract

Latar Belakang : Kejadian infeksi luka bedah menempati posisi ke 3, dengan angka kejadian mencapai 14-16% dari infeksi nosokomial keseluruhan. Angka kejadian infeksi luka bedah pada rumah sakit di Indonesia bervariasi antara 2-18% dari keseluruhan prosedur pembedahan. Kolagen memainkan peranan yang sangat penting di dalam pembentukan jaringan dan organ, terlibat di dalam berbagai fungsi biologis sel seperti pembelahan, pertahanan, dan diferensiasi sel. Karena fungsi biologis tersebut penggunaan kolagen dalam industri, khususnya dalam bidang medis, berkembang sangat pesat. Berdasarkan hasil penelusuran peneliti di Indonesia belum banyak penelitian terkait pemanfaatan kolagen ikan untuk penyembuhan luka, sehingga peneliti tertarik untuk melakukan penelitian pengaruh pemberian kolagen ikan terhadap kadar TGF ß1, jumlah fibroblas serta ketebalan kolagen dalam proses penyembuhan luka insisi pada tikus putih (Rattus norvegicus). Metode : 36 ekor tikus putih jantan Rattus novergicus galur wistar yang sehat berusia sekitar 3 bulan dengan berat sekitar 150-300 gram, kemudian dibagi menjadi 6 kelompok secara random. Rancangan penelitian yang digunakan adalah Randomised control group pre-post test design. Kolagen ikan diberikan sebanyak 0,09/200 kg bb melalui oral. Pengukuran kadar TGF ß1, dilakukan hari ke 0 (pre test) pada kelompok intervensi dan kontrol, selanjutnya dilakukan pengukuran kembali kadar TGF ß1 (post test), jumlah fibroblas, ketebalan kolagen pada hari ke 3,7 dan 10 pada kelompok intervensi dan kontrol. Hasil : Kolagen ikan bekerja maksimal pada kelompok tikus intervensi hari ke 3 dengan peningkatan kadar TGF ß1 secara signifikan dibandingkan kelompok kontrol (p<0,05), sedangkan pada hari ke 7 dan hari ke 10 tidak ada perbedaan signifikan kadar TGF ß1 antara kelompok intervensi dan kelompok kontrol (p>0,05). Hasil pemeriksaan histopatologi hari ke 3, 7 dan 10 menunjukkan perbedaan jumlah fibroblas dan ketebalan kolagen antara kelompok intervensi dan kelompok kontrol. Kesimpulan : Kolagen ikan berperan pada proses inflamasi dengan meningkatkan kadar TGF ß1, sedangkan pada fase proliferasi dan remodelling kolagen ikan berperan dalam angiogenesis, pembentukan extracellular matrix ECM dan sintesis kolagen. Kata kunci : kolagen ikan, TGF ß1, fibroblas, kolagen. Background: The incidence of surgical wound infection occupies position 3, with incidence rates reached 14-16% of nosocomial infections overall. The incidence of surgical wound infections in Indonesian hospitals varies between 2-18% of all surgical procedures. Collagen plays a very important role in the formation of tissues and organs are involved in various biological functions such as cell division, defense, and cell differentiation. Wherefore the biological function of the use of collagen in the industry, especially in the medical field, is growing very rapidly. Based on the search results of researchers in Indonesia have not been many studies related to the use of fish collagen for wound healing, so that researchers interested in conducting research the effect of fish collagen on levels of TGF-ß1, the amount of fibroblasts and thickness of collagen in wound healing incision in rats (Rattus norvegicus). Methods: 36 male rats of Rattus norvegicus Wistar strain are health about 3 months old, weighing approximately 150-300 grams, were divided into 6 groups randomly. The study design used was Randomised control group pre-post test design. Fish collagen is given as 0.09 / 200 kg bb by oral. Measurement of levels of TGF ß1, done day 0 (pre-test) in the intervention group and the control, then performed the re-measurement of TGF ß1 levels (post-test), the amount of fibroblasts, collagen thickness days 3, 7 and 10 in the intervention group and the control. Result: Fish collagen work optimally to rats intervention group on day 3 with increased levels of TGF-ß1 significantly than the control group (p <0.05), whereas on day 7 and day 10 there was no significant difference in the levels of TGF-ß1 between the intervention group and the group controls (p> 0.05). Histopathological examination results on day 3, 7 and 10 showed differences amount of fibroblasts and collagen thickness between the intervention group and the control group. Conclusion: Fish Collagen plays a role in inflammatory processes with increased levels of TGF-ß1, whereas in the proliferative phase and remodeling of collagen fish play a role in angiogenesis, the formation of extracellular matrix (ECM) and collagen synthesis.
