GAMBARAN PARTISIPASI DISCHARGE PLANNING PADA PASIEN PENYAKIT GINJAL KRONIK DI RSI SULTAN AGUNG SEMARANG

Abstract

Discharge Planning is a health service that starts with the patient to come to the hospital until return to the home. Discharge Planning has some factor like participation. This study aims to determine participation discharge planning in patients with chronic kidney disease. This type of research is descriptive non-experimental. The sampling technique uses purposive sampling. The number of samples in this study was 103 patients chronic kidney disease at RSI Sultan Agung Semarang. The research instrument used the P-QPD questionnaire to know participation discharge planning rates. The questionnaire has been tested for validity and reliability in RSUD Tugurejo Semarang, the results that the questionnaire is valid with calculated r value> (0.532) and reliable with Cronbach's Alpha score > 0.60. The data were analyzed by search mean value in each subscale. The results show chronic kidney disease patients have low participation in the P-Medical Treatment with mean value is 4.86 and P-Goals and Needs with Mean value is 13.37. Discharge planning participation is high in the P-Information with a mean value is 19.94. It is recommended for chronic kidney disease patients to participate in discharge planning and for health workers to provide opportunities for patients to participate in discharge planning.
