Gambaran Intervensi Discharge Planning Pada Pasien Hambatan Mobilitas Fisik Akibat Fraktur Ekstremitas Bawah Di RSI Sultan Agung Semarang Tahun 2017-2018

Abstract

Background: Discharge planning interventions in patients with impaired physical mobility due to fractures of the lower extremities are given to maintain and prepare patients' needs when they are discharged from the hospital. Current discharge planning interventions have not more been given in hospitals. Aim: to find out how the percentage of each discharge planning intervention in a patient impaired physical mobility due to a lower extremity fracture in the hospital. Methods: a document analysis study using the discharge planning intervention checklist instrument impaired physical mobility due to lower limb fractures. Sampling used was total sampling with a sample size of 288 medical records documents hospitalized with nursing diagnoses impaired physical mobility due to fractures of the lower extremities. Results: the percentage of the 228 nursing care documents each given intervention was 71.49% for the use of motion aids education, 16.67% for the intervention provided a motion aids, 53.07% for teaching patients how to change positions as indicated, 49.12 % for interventions consulted with physical therapists about ambulation exercises, 23.25% for patient training interventions to mobilize and the most intervention provided by nurses was educational intervention for muscle strengthening exercises that was 75.88%. Conclusion: Discharge planning in patients with impaired physical mobility due to fracture of the lower extremities has 6 interventions. Discharge planning intervention seen from the medical record documentation has not been 100% given by nurses and educational intervention on muscle strengthening exercises in patients is the largest intervention percentage compared to other interventions.
