Abstract

Congestive heart failure is one of cardiovascular diagnoses that increase rapidly and often requires re-treatment in hospital despite outpatient treatment has been given optimally. The average total of rehospitalization of patients with congestive heart failure reached 64%. One of factors that leads to the occurrence of rehospitalization is a lack of health education about the disease and how to conduct self-care at home given the nurse in the form of discharge planning activities. Training of discharge planning facilitates nurses to improve their capabilities in the implementation of discharge planning in patients with congestive heart failure. The purpose of this research was to prove that discharge planning training could prevent rehospitalization for patients with congestive heart failure. This research used quasi-experimental research design with posttest only design. This study was conducted in 46 respondents taken using consecutive sampling technique divided into the intervention group (n=23) and the control group (n=23). During the treatment, the respondents got discharge planning training and the occurrence of rehospitalization was assessed 30 days after discharge from the hospital. Data were obtained by observation and analyzed using the independent sample t-test. The results showed that discharge planning training was effective in reducing the occurrence of rehospitalization in congestive heart failure patients with p-value of 0.000. This study recommends the development of coaching to improve the implementation of discharge planning. Keywords : Discharge Planning, Congestive Heart Failure, Rehospitalization,
