Activities of the OSH program at a hospital can be reported periodically and systematically to a manager for making a decision accurately. The success of information system at an organisation depends on a method to apply a system, easiness to operate a system by users, and a technology used. Evaluation of a program is very important and beneficial particularly for a decision maker. This study aimed at evaluating OSH information system at Dr. Moewardi Public Hospital using a HOT Fit model. This was a descriptive study using a qualitative approach. Research subjects consisted of 8 person who were involved in information system management. In contrast, research object was OSH information system at Dr. Moewardi Public Hospital. Content analysis was performed to describe information system based on the HOT Fit model embraced Man, Organisation, and Technology. The results of this research showed that viewed from the factor of man, number of officers was insufficient. There was no officer who had an educational background of OSH or expert in OSH. In addition, there was no specific training of OSH information system. Regarding the factor of organisation, structure of organisation in the form of a committee was available. There was no specific funds for OSH. Regarding the factor of technology, quality of the system was good enough. However, in some units, a form was filled manually. Quality of information was good. Quality of service was good enough. Means and infrastructure were limited due to sharing with other units. The hospital management needs to conduct training of the use of OSH information system for officers, to make a plan of specific funds for OSH information system, to add officers that are adjusted with their education and the necessity, and to provide a specific room for OSH unit at the hospital.
