Abstract

Latar Belakang : Dengue Shock Syndrome (DSS) merupakan fase III dan IV pada demam akibat infeksi dengue, yang dapat berakibat pada keparahan penyakit dan berujung kematian Tahun 2016 jumlah kasus DBD/DSS di Kota Semarang sebanyak 2.200 kasus (IR=124,50) dengan jumlah kasus DSS sebanyak 293 kasus atau 15,4% dari kasus DBD dan CFR DSS sebesar 9,21%. Tujuan : Tujuan penelitian untuk membuktikan beberapa variabel (karakteristik klinis host dan sosiodemografik) yang berpengaruh terhadap kejadian dengue shock syndrome. Metode : Penelitian observasional analitik menggunakan desain studi kasus kontrol. Populasi dalam penelitian adalah pasien dengan diagnosis DSS atau diagnosis DBD yang dirawat di RSUD K.R.M.T Wongsonegoro Kota Semarang. Sampel penelitian 70 kasus dan 70 kontrol dengan teknik consecutive sampling. Data dianalisis dengan regresi logistik. Hasil : Pasien DBD dengan umur = 15 tahun OR=4,277 (95% CI=1,325-13,598), infeksi ulangan OR=2,807 (95% CI=1,116-7,055), hepatomegali OR=3,206 (95% CI=1,055-9,745), dan trombositopenia (<50.000/µL) OR=11,301 (95% CI=4,67427,325) merupakan faktor yang berpengaruh terhadap kejadian dengue shock syndrome. Sedangkan jenis kelamin, mual/muntah, hemokonsentrasi, IMT, golongan darah, lama mendapatkan pelayanan kesehatan, tanda perdarahan spontan, nyeri abdominal, hipotensi, akses menuju pelayanan kesehatan, pengetahuan dan kesadaran orangtua/penderita tidak berpengaruh dengan kejadian DSS Simpulan : Karakteristik klinis host yang terbukti berpengaruh terhadap DSS adalah umur = 15 tahun, infeksi ulangan, hepatomegali, dan trombositopenia (<50.000/µL). Study populations were patients with diagnosis of DSS or diagnosis of DHF that admitted in K.R.M.T Wongsonegoro Hospital Semarang City. The samples were 70 cases and 70 controls by consecutive sampling. Data were analyzed by logistic regression. Sex, nausea/vomiting, hemoconcentration, BMI, blood type, prolonged acces to health services, signs of spontaneous bleeding, abdominal pain, hypotension, acces to health services, knowledge and awareness of parents / suffers were not associated with DSS Conclusion: Host clinical characteristic variables were evident influenced to DSS is age = 15 years, secondary infection, hepatomegaly, and thrombocytopenia (<50.000/µL).
