Abstract
The Discharge Planning is a process of the professional interaction and collaboration between health teams, patients and the patients’ families to provide and manage the continuity of a patients’ medical care. The discharge planning analysis, diagnose, plan and evaluation are necesary to be conducted since this research have not been condudcted yet in the inpatient wards of RSUD Ungaran. Thus, there is an increasing of LOS (Length of Stay) requiring the patient to stya longer. This non experimental quantitative research with descriptive research design aimed to determine the description of the nurses’ perseption in the implementation of discharge planning in the inpatient wards of RSUD Ungaran. This research used Proportional Random Sampling technique which was undertaken on 97 respondents using questionnaire and then were analyzed using descriptive statistic. The results showed that the role of nurses in implementation discharge planning were as a good assessment (67.0%), a good diagnosis (61.9%), a good planning (64.9%), a good implementation (67.0%) and a good evaluation (60.8%). In short, the role of nurses in the implementation of discharge planning are good (51.5%), therefore the nurses need to maintain its role in order to enhance the quality of documentation discharge planning. To sum up, the Implementation of the nurses’ perseption can be optimized by providing a training, seminars, evaluation and supervision.
