Abstract

The Influence of Deep Breathing Exercise and Active Range of Motion on Dyspnea and Physiological Response in Patients with Congestive Heart Failure xviii +160 pages + 39 tables + 4 figures + 5 charts + 15 appendixes Dsypnea is a clinical manifestation of CHF due to pulmonary function failure. The accumulation of fluid in the alveoli makes the heart unable to pump up to the maximum. Changes that occur in the respiratory muscles make an increase of the sensation of dyspnea. This results in increased respiratory rate, blood pressure, pulse and decreased oxygen saturation, which become important factors affecting the quality of life of patients. Non-pharmacological management is performed to mantain physical stability, avoid behaviors that can worsen the condition and detect early symptoms of worsening heart failure. Deep breathing exercise and active range of motion is one of the interventions to reduce breathing effort and inscrease respiratory muscles. The purpose of this study was to analyze the influence of deep breathing exercise and active range of motion on dyspnea and physiological response in CHF patients. This study was an experimental research with pretest-posttest control group design with 32 respondents recruited by stratified random sampling technique. Intervention began with deep breathing exercise 30 times and continued with active range of motion with each movement of five times. Intervention was done 3 times a day for 3 days. The measuring instruments in this research were Modified Borg Scale, digital spygnomanometer and pulse oximetry. The time of research was from April to July 2017 in RS PKU Muhammadiyah Yogyakarta and RS PKU Muhammadiyah Gamping Yogyakarta. Data analysis used was independent test t-test and Mann-Whitney. Results of the mean difference between the two grup found a p value for each variable i.e, dyspnea = 0.004, systolic = 0.003, diastole = 0.296, pulse = 0.542, respiratory rate = 0.184, and oxygen saturation = 0.019. Interventions of deep breathing exercise and active range of motion can decrease dyspnea and systolic blood pressure and increased oxygen saturation in CHF patients. Further research is expected to increase the time of intervention to allow for better significance.
