Abstract
Latar Belakang: Nusa Tenggara Barat merupakan salah satu provinsi di Indonesia yang memiliki prevalensi stunting (48,3%) lebih tingi dari angka nasional (37,2 %). Di Indonesia masih ada tradisi pemberian makanan prelakteal dan makanan papahan pada bayi, pengaruh kebiasaan ini pada stunting belum diketahui. Tujuan: Membuktikan bahwa pemberian makanan prelakteal dan pemberian makanan papahan merupakan faktor risiko kejadian stunting pada anak usia 12-24 bulan dengan mengendalikan faktor risiko lain yang berperan sebagai perancu. Metode: Penelitian observasional dengan rancangan kasus kontrol pada 64 kasus (stunting) dan 64 kontrol (normal) di Kecamatan Sakra Timur. Subyek dipilih menggunakan metode simple random sampling dengan cara di undi. Instrumen pengumpulan data menggunakan kuesioner terstruktur dan pengukuran panjang badan menggunakan infantometer. Analisis data dilakukan secara bertahap meliputi analisis univariat, bivariat (chi-square) dan multivariate (regresi logistik). Hasil: Dari hasil uji bivariat ada empat variabel yang memiliki hubungan bermakna dengan kejadian stunting yaitu tingkat kecukupan energi (p=0,031), tingkat kecukupan protein (p=0,034), tidak ASI eksklusif (p=0,01), kejadian diare (p=0,021) dan pemberian makanan prelakteal (p=0,001). Pemberian makanan prelakteal merupakan faktor risiko kejadian stunting (OR=3,65; 95% CI=1,7-7,5; p=0,001) setelah dikendalikan dengan tingkat kecukupan energi dan diare. Pemberian makanan papahan bukan merupakan faktor risiko terhadap kejadian stunting dalam penelitian ini. Simpulan: Pemberian makanan prelakteal merupakan faktor risiko kejadian stunting Saran: Perlu adanya peningkatan promosi kesehatan oleh petugas kesehatan tentang manfaat ASI eksklusif dan bahaya pemberian makanan prelakteal untuk anak. Background: West Nusa Tenggara is one of the provinces in Indonesia which has prevalence of stunting (48.3%) higher than the national rate (37.2%). In West Nusa Tenggara, tradition of prelacteal feeding and pre-swallowed feeding (makanan papahan) in infants still exist, but the influence of this habit on stunting has not been known. Objective: To show that prelacteal feeeding and pre-swallowed feeding (makanan papahan) were the risk factors of stunting among 12-24 month old children in East Sakra Subdistrict, District of East Lombok, Province of West Nusa Tenggara by controlling with the confounders. Methods: The study was conducted in a case-control design, with 64 cases (stunting) and 64 controls (normal) in East Sakra Subdistrict. Subject were selected using systematic random sampling method. Data were collected by interviews using structured questionnaires and body length were measured by infantometer. Data were analysed bivariately by chi-square test and multivariately by logistic regression. Result: Variables wich showed the association with stunting were low energy adequacy (p = 0.031), low protein adequacy (p = 0.034), non exclusive breastfeeding (p = 0.01), the incidence of diarrhea (p = 0.021) and prelakteal feeding (p = 0.001). Prelacteal feeding was the risk factor of stunting (OR=3,65; 95% CI=1,7-7,5; p=0,001) after controlled by energy adequacy and diarrhea. Pre-swalowed feeding (makanan papahan) was not the risk factor to the incidence of stunting in this study. Conclusion: Prelacteal feeding was the risk factor of stunting Suggestions: It is recommended to increase health promotion about the benefits of exclusive breastfeeding and the dangers of prelacteal feeding on neonates.
