Abstract

Program Studi Magister Epidemiologi Program Pasca Sarjana Undip 2007 ABSTRAK Yusnidar Faktor-Faktor Risiko Penyakit Jantung Koroner Pada Wanita Usia > 45 Tahun (Studi Kasus di RSUP Dr. Kariadi Semarang) xix + 101 halaman + 38 tabel + 3 grafik + 9 bagan + 6 lampiran Latar belakang : Suatu penelitian pada tahun 2002 menunjukkan bahwa 28% dari seluruh wanita yang berusia diatas 50 tahun meninggal karena PJK, sehingga menjadi penyebab utama kematian wanita dalam kelompok umur tersebut. Usia >45 tahun merupakan masa peralihan dari premenopause ke perimenopause, sehingga sangat penting dilakukan pendekatan gender spesific tentang faktor-faktor risiko PJK. Tujuan : Untuk mengetahui berapa besar pengaruh faktor-faktor risiko terhadap kejadian penyakit jantung koroner (PJK) pada wanita usia >45 tahun. Metode : Jenis penelitian adalah observasional dengan rancangan kasus kontrol. Jumlah sampel 78 kasus dan 78 kontrol. Kasus adalah pasien penyakit jantung koroner yang dirawat di Unit Penyakit Jantung RSUP Dr. Kariadi Semarang yang diperoleh dari data rekam medis yang memenuhi kriteria inklusi dan eksklusi. Kontrol adalah pasien penyakit kardiovaskuler yang bukan penyakit jantung koroner yang dirawat di di Unit Penyakit Jantung RSUP Dr. Kariadi Semarang. Hasil : Analisis multivariat menunjukkan bahwa faktor-faktor yang terbukti berpengaruh terhadap kejadian PJK dan merupakan faktor risiko PJK pada wanita usia >45 tahun adalah: menopause (OR=7,2; 95% CI 2,1-24,8); penuaan (kelompok umur ³66 tahun) (OR=6,0; 95% CI 2,1-17,2); inaktivitas fisik (lama duduk ³3,25 jam/hari) (OR=4,1; 95% CI 1,7-9,9); riwayat diabetes mellitus (OR=3,9; 95% CI 1,6-9,6); riwayat hipertensi (OR=3,5; 95% CI 1,6-7,8); dan tingkat pengetahuan (OR=2,4; 95% CI 1,1-5,3). Saran : Wanita sebaiknya melakukan aktivitas fisik intensitas sedang (misalnya berjalan cepat) minimal 30 menit terus-menerus perhari setiap hari. Modifikasi gaya hidup dan farmakoterapi harus dilakukan pada wanita yang mempunyai faktor risiko seperti diabetes dan hipertensi. Pemberian aspirin rutin dapat dipertimbangkan pada wanita usia >65 tahun jika keuntungan (menurunkan risiko serangan jantung dan stroke) lebih besar daripada risiko kemungkinan perdarahan. Menopausal therapy, suplementasi antioksidan, asam folat (dengan atau tanpa suplementasi B6 dan B12), dan penggunaan aspirin rutin pada wanita usia <65 tahun sebaiknya tidak dilakukan untuk prevensi penyakit kardiovaskuler primer atau sekunder. Penyuluhan dan penelitian lanjutan tentang penyakit kardiovaskuler (prevensi dan terapi) seharusnya menjadi prioritas. Over the age of 45 is a transitional time from premenopausal to perimenopausal, a gender specific approach should be used to find out the risk factors of CHD. Objective : The objective was to determine the influence of risk factors to the occurrence of CHD in women aged >45 y. Methods : This was an observational study using case-control design. The number of samples were 78 cases and 78 controls. Cases were patients of CHD admitted in the cardiology care unit Dr. Kariadi Hospital Semarang, selected from the medical records that fulfilled inclusion and exclusion criteria. Controls were patients with non-CHD that admitted in the cardiology care unit Dr. Kariadi Hospital Semarang. Results : Factors that influence CHD event and risk factors for women aged >45 y were menopause (OR=7,2; 95% CI 2,1-24,8); aging (³66 y) (OR=6,0; 95% CI 2,1-17,2); physical inactivity(sitting time ³3,25 hr/d) (OR=4,1; 95% CI 1,7-9,9); history of diabetes mellitus (OR=3,9; 95% CI 1,6-9,6); history of hypertension (OR=3,5; 95% CI 1,6-7,8); and knowledge of heart disease (OR=2,4; 95% CI 1,1-5,3). Suggestions : Women should accumulated a minimum of 30 minutes of moderate-intensity physical activity (eg. brisk walking) on most or all days of the week. Lifestyle modification and pharmacotherapy should be used as indicated in women with diabetes and hypertension. Routine use of aspirin in women aged >65 y should be considered, if the benefits (for ischemic MI and stroke) is likely to overweigh risk of bleeding. Menopausal therapy, antioxidant supplements, folic acid, and aspirin for MI in women <65 years of aged should not be used for the primary or secondary prevention of CVD. Health promotion about CVD (prevention and therapy) should be priority conducted.
