Abstract
Universitas Diponegoro Fakultas Kesehatan Masyarakat Program Studi Magister Ilmu Kesehatan Mayarakat Konsentrasi Administrasi dan Kebijakan Kesehatan 2016 ABSTRAK Auva Rovkoti Analisis Implementasi Program Jaminan Kesehatan Nasional Dalam Pelayanan Kesehatan Gigi dan Mulut pada Fasilitas Kesehatan Tingkat Pertama di Kabupaten Demak. XV + 115 halaman + 5 tabel + 9 gambar + 14 lampiran Keterbatasan sarana prasarana dan tenaga dalam jumlah maupun kemampuan petugas pelayanan kesehatan gigi dan mulut di FKTP Kabupaten Demak menyebabkan terbatasnya pula pelayanan yang dilakukan. Penelitian ini bertujuan menganalisis implementasi program JKN dalam pelayanan kesehatan gigi dan mulut pada Fasilitas Kesehatan Tingkat Pertama (FKTP) di Kabupaten Demak Penelitian ini adalah penelitian observasional dengan rancangan kualitatif yang disajikan secara deskriptif melalui wawancara mendalam dan observasi. Informan utama ada 11 orang terdiri dari 4 perawat gigi, 3 dokter gigi puskesmas, 3 dokter gigi keluarga dan 1 dokter gigi keluarga merangkap dokter gigi puskesmas. Informan triangulasi berjumlah 8 orang yaitu : 4 kepala puskesmas, 1 dokter gigi spesialis di rumah sakit, 1 ketua PDGI cabang Semarang, 1 kepala BPJS KCU Semarang. Analisis data dilakukan dengan analisis isi. Hasil penelitian menunjukkan bahwa pelayanan kesehatan gigi dan mulut dalam program JKN ini dapat dibagi menjadi tiga kelompok yaitu pelayanan pada FKTP dokter gigi keluarga, pelayanan pada FKTP puskesmas dengan dokter gigi, pelayanan pada FKTP puskesmas tanpa dokter gigi. Transmisi komunikasi dan kejelasan informasi pada FKTP masih kurang terutama pada FKTP puskesmas tanpa dokter gigi. Konsistensi komunikasi juga perlu diperbaiki. Adanya dana kapitasi dari BPJS lebih memudahkan untuk melengkapi sarana dan prasarana serta meningkatkan pendapatan para petugas, namun dana kapitasi ini belum dapat digunakan secara optimal pada FKTP puskesmas. Terkait dengan ketersediaan tenaga masih ada kekurangan yang mengakibatkan terganggunya pelayanan yang diberikan. Sikap dan komitmen petugas sudah baik namun membutuhkan penyempurnaan lebih lanjut dan petugas menjalankan tugas pokok dan fungsi (tupoksi) mereka. Pelaksanaan pelayanan kesehatan gigi mulut di FKTP kabupaten Demak ini masih menggunakan SOP yang belum disesuaikan dengan program JKN. Disimpulkan bahwa implementasi program JKN dalam pelayanan kesehatan gigi dan mulut pada pasien di FKTP Kabupaten Demak belum optimal. Bagi Dinas Kesehatan Kabupaten Demak dan BPJS hendaknya lebih mendorong petugas untuk meningkatkan pengetahuan dan ketrampilan, membuat regulasi pemenuhan tenaga kesehatan, menjalankan fungsi monitoring pelaksanaan pelayanan serta memperjelas aturan dan batas pelayanan di FKTP. The aim of this study was to analyse the implementation of a National Health Insurance (NHI) program in providing dental and oral health services at the FLHF in Demak District. This was an observational study using a qualitative method presented descriptively by conducting indepth interview and observation. Number of main informants were 11 persons consisted of four dental hygienists, three health dentists, three family dentists, and a family and health centre dentist. Number of informants for triangulation purpose were eight persons consisted of four heads of health centres, a dental specialist at a hospital, a head of the Indonesian Dental Association of Semarang Branch, and a head of BPJS of Semarang Branch. Data were analysed using content analysis. The results of this research showed that dental and oral health services in the NHI program were divided into three groups, namely services at the FLHF with a family dentist, services at the FLHF with a health centre dentist, services at a health centre without a dentist. There was a lack of communication and clarity of information at the FLHF of a health centre particularly at a health centre without a dentist. Consistency of communication needed to be improved. The availability of capitation fund from BPJS eased in providing means and in increasing officers’ income. Notwithstanding, the capitation fund could not be used optimally by health centres. The provided services were not optimal due to insufficient number of officers. Attitude and commitment of officers were good. However, their main tasks and functions needed to be improved. Standard Operating Procedure was not in accordance with the NHI program. To sum up, the implementation of the NHI program at the FLHF in Demak District was not optimal. Demak District Health Office and BPJS need to support officers to improve their knowledge and skills, to make a regulation for recruiting health workers, to monitor the implementation of services, and to explain regulations and limitations of services at the FLHF.
