For that reason, a discharge planning is conducted by the hospital health promotion team. Handling of stroke occurs at the acute and rehabilitation phase. In the process of stroke rehabilitation, home caregiving experience will pose some burdens on the caregivers. This study aimed to determine the associations between discharge planning in the implementation of hospital health promotion of Tugurejo public hospital, Semarang, and the burden of caregivers of post-stroke patients in Semarang. The method used in this study was an analytical cross-sectional survey. The samples were the caregivers of post-stroke patients who had returned home from the inpatient wards of Tugurejo hospital in January-February 2016. In total, the samples were 36 caregivers who were determined by a total sampling technique. A documentation study was conducted on the completeness of the discharge planning forms including the documents of standard operating procedures of discharge planning interventions. The data were analyzed using a chi-square test. The results showed an association between discharge planning in the implementation of hospital health promotion of Tugurejo public hospital and the burden of caregivers of post-stroke patients in Semarang (p = 0.004; α = 0.05). The perceived burden was multidimensional stress as evident in the caregivers themselves.
