Universitas Diponegoro Fakultas Kesehatan Masyarakat Program Studi Magister Ilmu Kesehatan Masyarakat Konsentrasi Administrasi dan Kebijakan Kesehatan 2016 ABSTRAK Rizal Purnomo Analisis Dampak Kebijakan: Penerapan Pola Pengelolaan Keuangan Badan Layanan Umum Daerah di Puskesmas Kabupaten Tegal xix + 250 halaman + 13 tabel + 5 gambar + 10 lampiran Puskesmas memiliki fungsi untuk memberikan pelayanan kesehatan dasar kepada masyarakat. Kebijakan penerapan Puskesmas sebagai Badan Layanan Umum Daerah (BLUD) diimplentasikan untuk meningkatkan kualitas pelayanan kesehatan dasar. Kebijakan penerapan Pola Pengelolaan Keuangan BLUD telah diltetapkan kepadasemua Puskesmas di Kabupaten Tegalsejak tahun 2015.Sebuah kebijakan baru dapat menimbulkan dampak segera maupun jangka panjang. Tujuan penelitian ini untuk menganalisis penerapan Pola Pengelolaan Keuangan BLUD, menganalisis dampak kebijakan, serta upaya yang dilakukan untuk mengatasi dampak negatif yang tidak diharapkan dari kebijakan penerapan pola pengelolaan keuanganbadan layanan umum daerahdiPuskesmas Kabupaten Tegal. Penelitian ini merupakan penelitian evaluasi dengan pendekatan kualitatif. Subjek penelitian Puskesmas di Kabupaten Tegal yang melaksanakan kebijakan penerapan pola pengelolaan keuangan BLUDdi Puskesmas. Data kualitatif dikumpulkan dengan metode wawancara mendalam.Pengolahan dan analisis data menggunakan content analysis. Hasil penelitian menunjukkan kinerja puskesmas sebelum dan sesudah kebijakan penerapan PPK BLUD di Puskesmas menunjukkan trend yang baik. Dampak kebijakan yang terjadi adalahperubahan budaya kerja, peningkatan penghasilan pegawai, puskesmas mempunyai kemandirian dalam pengelolaan anggaran, pengembangan program inovatif, dan terjadi peningkatan kualitas pelayanan. Dampak negatif yang tidak diharapkan terjadi peningkatan beban kerja tenaga pengelola BLUD, puskesmas belum bisa menggunakan dana surplus, tenaga non PNS, dan sistem remunerasi belum dapat dilaksanakan karena puskesmas gratis. Disarankan untuk mengikutkan staf pengelola PPK BLUD mengikuti Pelatihan pengelolaan keuangan, perlu adanya perubahan kebijakan penggunaan dana surplus puskesmas, pemberian dana operasional sebagai kompensasi pembebasan biaya pelayanan kesehatan di Puskesmas, membentuk tim pengelola BLUD, dan inovasi pelayanan puskesmas untuk menggali pendapatan puskesmas. Determination of a health centre as a regional public service agency (RPSA) is to improve quality of basic health service. The policy has been applied to all health centres in Tegal District since 2015. A new policy can cause either short or long-term effects. The aim of this study was to analyse the implementation of financial management pattern of RPSA, to analyse effects of the policy, and to overcome negative effects of the policy in Tegal District. This was an evaluation study using qualitative approach. Research subjects were health centres in Tegal District that applied the policy of financial management pattern of RPSA. Data were collected by conducting indepth interview and analysed using a method of content analysis. The results of this study showed that the performance of health centres after implementing the policy tended to be better. Some positive effects were as follows: changes in work culture, improvement of officers’ income, independent in managing funds, development of innovative program, and improvement of service quality. Some negative effects were as follows: the increase in workload of RPSA administration officers, unused surplus funds at health centres, non civil servant officers, and no implementation of remuneration system at health centers due to free of charge. Administration staffs of RPSA need to follow training of financial management. In addition, there needs to change the policy of using surplus funds and to provide operational funds as compensation of service fee waiver at health centres. A RPSA team needs to be formed and innovation of services needs to be developed for increasing income of health centres.
