Abstract

Background Drug of choice on antihypertensive therapy is influenced by various factors, one of them is whether there is a compelling indication or not in hypertensive patients. Aim to Know the pattern of antihypertensive therapy on outpatients in cardiovascular care unit dr. Kariadi Hospital Semarang on January 2015. Method This research is a descriptive study with a sample of 171 people. Datas were collected by recording basic informations and prescribed antihypertensive drugs on the medical records in January 2015 by simple random sampling. Prevalence study was using nine days ( 5, 7, 9, 13, 19, 21, 23, 27, 29) on January 2015. Results The prevalence of hypertension in outpatient of cardiovascular care unit in Dr.Kariadi Hospital Semarang is 54.06%. The prevalence of hypertension with compelling indication was 53.86% and the prevalence of hypertension without compelling indication was 0.19%. Old age subject (≥60 years) (60.20%) and adult (18-59 years) (39.80%). Based on Gender, men (65.5%), while the women (34.5%), The mean systolic blood pressure was 130 (100-178) and mean diastolic blood pressure was 81.43 (60-118). Compelling indication heart failure (59.05%) and IHD (40.35%) . Subjects with compelling indicaton was 99.40%. Subjects without compelling indication was 0.60%. Antihypertensive drug classes which frequetly used were ARB (69.00%), BB (64.90%), CCB (35.70%). Pattern of antihypertensive therapy in subjects with heart failure, Monotherapy BB (6.90%), 2-drug combination therapy ARB + B (15.70%), 3-drug combination therapy ARB + BB + CCB (11.80%), 4-drug combination therapy ARB + BB + diuretics + aldosterone inhibitor (2.00%). Pattern of antihypertensive therapy in subjects with IHD, monotherapy ARB (13.20%), 2-drug combination therapy ARB + B (23.50%), 3-drug combination therapy ARB + BB + CCB (16.20%). Conclusion The pattern of antihypertensive therapy was similar in compelling indications found in the study (heart failure and IHD).
