Universitas Diponegoro Fakultas Kesehatan Masyarakat Program Magister Ilmu Kesehatan Masyarakat Konsentrasi Kesehatan Ibu dan Anak 2015 ABSTRAK Lusiana Meinawati Faktor Determinan Pemanfaatan Buku Pedoman Motivator ASI oleh Kader di Puskesmas Wilayah Kerja Kabupaten Jombang Tahun 2015 xvi + 92 halaman + 19 tabel + 10 lampiran Angka kematian bayi di Kabupaten Jombang meningkat dari 12,15/1000 KH pada tahun 2012 menjadi 13,18/1000 KH pada tahun 2013. Pemberian ASI eksklusif terbukti menurunkan angka kematian bayi, sehingga motivator ASI sangat diperlukan. Pemanfaatan buku pedoman motivator ASI masih belum optimal. Tujuan penelitian untuk menganalisis berbagai faktor yang mempengaruhi pemanfaatan buku pedoman oleh kader sebagai motivator ASI di beberapa Puskesmas Wilayah Kerja Kabupaten Jombang. Jenis penelitian analitik dengan pendekatan cross sectional. Pengumpulan data dilakukan dengan wawancara menggunakan kuesioner terstruktur. Populasi adalah kader motivator ASI sejumlah 245 kader. Jumlah subjek 100 kader yang dipilih secara simple random sampling. Analisis data dilakukan dengan Chi Square untuk analisis bivariat dan regresi logistik ganda untuk multivariate. Hasil penelitian menunjukkan bahwa rata – rata umur subjek 35 tahun, dan rata – rata lama menjadi kader selama 3 tahun, sebanyak 64% subjek berpendidikan <9 tahun dan 52% subjek mendapatkan pelatihan 1 kali. Sebanyak 92% subjek berpengetahuan baik, 56% subjek mempunyai sikap positif, 82% subjek mendapatkan pembinaan, 56% subjek tidak mendapatkan dukungan dari tokoh masyarakat, 81% subjek mendapatkan supervisi, dan 81% subjek memanfaatkan buku pedoman. Variabel yang berhubungan dengan pemanfaatan yaitu sikap kader (p=0,0001), pembinaan (p=0,0001), dukungan toma (p=0,040), supervisi (p=0,0001). Variabel yang paling berpengaruh terhadap pemanfaatan buku pedoman adalah pembinaan ((p= 0,007), EXP (B) = 11,566) Bagi puskesmas perlu dilakukan penguatan program, evaluasi materi dalam buku pedoman, peningkatan kinerja bidan desa sebagai pendamping kader, serta perumusan pemberian reward bagi kader berprestasi. Kader motivator ASI diharapkan meningkatkan motivasi untuk tetap aktif dan memanfaatkan ketersediaan buku pedoman. Providing breast milk significantly decreased IMR. Therefore, motivator of breast milk was needed. A motivator guidance book of breast milk had not been optimally used. The aim of this study was to analyse factors influencing the use of guidance book by cadre as a motivator of breast milk at health centres in work area of Jombang District. This was an analytic study using cross-sectional approach. Data were collected by interviewing using structured questionnaires. Number of population was 245 cadres. As many as 100 cadres were selected randomly using a technique of simple random sampling. Data were analysed using analyses of bivariate (Chi Square test) and multivariate (logistic regression test). The results of this research showed that average age of the respondents was 35 years old, average working as cadre was 3 years, as many as 64% of the respondents had educational level less than 9 years attending school, and as many as 52% of the respondents had received training at least once. In addition, most of them had good knowledge (92%), had positive attitude (56%), had been guided (82%), were not supported by community leaders (56%), had been supervised (81%), and had used a guidance book (81%). Variables of cadre’s attitude (p=0.0001), development (p=0.0001), supporting from community leaders (p=0.040), supervision (p=0.0001) had significant relationship with the use of a guidance book. Guidance was the most dominant variable influencing the use of a guidance book ((p= 0.007); Exp(B)= 11.566). Health Centres need to strengthen a program, evaluate the content of the guidance book, improve work performance of village midwife as cadre’s supervisor, and arrange providing rewards for best cadre.
