Abstract
Universitas Diponegoro Fakultas Kesehatan Masyarakat Program Magister Ilmu Kesehatan Masyarakat Konsentrasi Administrasi dan Kebijakan Kesehatan Minat Manajemen Kesehatan Ibu dan Anak 2013 ABSTRAK Natiqotul Fatkhiyah Faktor Individu dan Organisasi yang Berpengaruh terhadap Kepatuhan Bidan Praktik Mandiri dalam Deteksi Preeklampsia di Kabupaten Tegal Provinsi Jawa Tengah Tahun 2013 xv + 82 Halaman + 17 tabel + 9 gambar + 10 lampiran Kejadian preeklampsia/eklampsia sebagai penyebab kematian maternal di Kabupaten Tegal dari tahun 2010-2012 cenderung mengalami peningkatan yang kemungkinkan disebabkan oleh kepatuhan. Rendahnya kepatuhan dapat dipengaruhi oleh faktor individu dan organisasi. Tujuan penelitian ini adalah menganalisis faktor individu (masa kerja, pengetahuan dan motivasi) dan faktor organisasi (kualitas supervisi dan persepsi penghargaan&sanksi) yang berpengaruh terhadap kepatuhan Bidan Praktik Mandiri (BPM) dalam deteksi preeklampsia di Kabupaten Tegal. Jenis penelitian observasional kuantitatif dengan pendekatan cross sectional. Variabel bebas meliputi masa kerja, pengetahuan, motivasi, kualitas supervisi serta persepsi penghargaan dan sanksi. Variabel terikatnya adalah kepatuhan dalam deteksi preeklampsia. Jumlah sampel 75 bidan praktik mandiri dipilih secara proportional. Pengumpulan data dengan kuesioner terstruktur dan observasi langsung. Data dianalisis secara kuantitatif dengan uji korelasi Chi-square dan regresi logistik. Hasil penelitian menunjukkan BPM yang patuh dalam deteksi preeklampsia sebesar 69,30% responden, masa kerja <8 tahun 50,70%, responden berpengetahuan baik sebesar 54,70%, bermotivasi tinggi sebesar 70,70% BPM, kualitas supervisi yang baik oleh Bidan Koodinator sebanyak 61,30% dan memiliki persepsi penghargaan dan sanksi yang baik sebesar 57,30% BPM. Faktor yang berhubungan dengan kepatuhan dalam deteksi preeklampsia adalah pengetahuan (ρ=0,002), motivasi (ρ=0,001) dan kualitas supervisi (ρ=0,0001). Secara bersama-sama (pengetahuan, motivasi dan kualitas supervisi) berpengaruh terhadap kepatuhan BPM dalam deteksi preeklampsia. Faktor yang paling dominan berpengaruh terhadap kepatuhan secara berurutan adalah kualitas supervisi (Exp(B)=8,129), motivasi (Exp(B)=7,167) dan pengetahuan (Exp(B)=4,291). Disarankan pada Dinas Kesehatan dan organisasi IBI Kabupaten Tegal untuk melaksanakan supervisi fasilitatif secara berkala dan komprehensif terhadap bidan praktik mandiri sebagai upaya peningkatan kepatuhan bidan dalam deteksi preeklampsia. Adanya upaya perbaikan dan monitoring evaluasi bagi BPM diharapkan terjadi peningkatan mutu deteksi preeklampsia. The low obedience was because of individual and organizational factors. This research aimed to analyze individual factors (working period, knowledge, and motivation) and organizational factors (quality of supervision, and perceptions of reward and punishment) influencing the obedience of privately practicing midwives (PPM) in detecting preeclampsia in Tegal District. This was observational analytic research with cross-sectional approach. Independent variables consisted of working period, knowledge, motivation, quality of supervision, and perceptions of reward and punishment. A dependent variable was the obedience in detecting preeclampsia. Number of samples was 75 privately practicing midwives taken proportionally. Data were collected using a structured questionnaire and direct observation. Furthermore, data were analyzed quantitatively using Chi-Square and Logistic Regression test. The results of this research revealed that mostly respondents obeyed in detecting preeclampsia (69.30%), had working period < 8 years (50.70%), had good knowledge (54.70%), and had high motivation (70.70%). In addition, most of them had been well supervised by midwife coordinators (61.30%) and had good perceptions of reward and punishment (57.30%). Factors of knowledge (p=0.002), motivation (p=0.001), and quality of supervision (p=0.0001) had relationships with the obedience of PPM in detecting preeclampsia. Furthermore, the factors jointly influencing the obedience consecutively were quality of supervision (Exp(B)=8.129), motivation (Exp(B)=7.167), and knowledge (Exp(B)=4.291). As a suggestion, Tegal District Health Office and Indonesian Midwives Association need to supervise PPM regularly as an effort to increase the obedience of them in detecting preeclampsia. Additionally, they also need to monitor and evaluate for improving quality of preeclampsia detection.
