Abstract

Universitas Diponegoro Fakultas Kesehatan Masyarakat Program Magister Ilmu Kesehatan Masyarakat Konsentrasi Administrasi dan Kebijakan Kesehatan Minat Manajemen Kesehatan Ibu dan Anak 2013 ABSTRAK Murti Wuryani Analisis Manajemen Pelaksanaan Imunisasi oleh Bidan Desa Kaitannya dengan Pencapaian Universal Child Imunization (UCI) di Kabupaten Batang xiii + 93 halaman + 15 tabel + 2 gambar + 9 lampiran Pencapaian UCI desa di Kabupaten Batang tahun 2011 sebesar 67% dan 2012 sebesar 87%, kondisi ini masih dibawah target SPM UCI Desa Jawa Tengah yaitu 90% yang diharapkan pada tahun 2014 pencapaian UCI 100%, hal ini menunjukkan terdapat kesenjangan pencapaian UCI, yang diduga belum optimalnya manajemen pelaksanaan imunisasi. Tujuan penelitian adalah menganalisis manajemen pelaksanaan imunisasi oleh bidan desa kaitannya dengan pencapaian UCI di Kabupaten Batang. Penelitian ini merupakan penelitian observasional dengan pendekatan cross sectional. Instrumen penelitian menggunakan kuesioner terstruktur dan checklist observasi terhadap kegiatan imunisasi. Jumlah sampel 72 bidan desa yang tersebar di 10 Puskesmas Kabupaten Batang. Analisis bivariat dengan uji chi square dan analisis multivariat dengan uji regresi logistik. Hasil penelitian menunjukkan proporsi bidan yang memiliki perencanaan baik 70%, pengorganisasian baik 87,5%, penggerakan baik 80% dan pengawasan baik 72,5% pada desa tercapai UCI sedangkan pada desa yang tidak tercapai UCI proporsi bidan yang memiliki perencanaan baik 40,6%, pengorganisasian baik 65,6%, penggerakkan 53,1% dan pengawasan 37,5%, pencapaian UCI yang sudah mencapai target 56,6%. Ke empat manajemen pelaksanaan imunisasi bidan desa semuanya berhubungan dengan pencapaian UCI yaitu perencanaan (p=0,012), pengorganisasian (p=0,027), penggerakan (p=0,015) dan pengawasan (p=0,003). Hasil multivariat menunjukkan bahwa manajemen pelaksanaan imunisasi yang berpengaruh adalah perencanaan (p=0,032) dan penggerakkan (p=0,041). Dalam rangka meningkatkan target UCI disarankan kepada bidan desa yang belum tercapai UCI agar lebih memperhatikan dan meningkatkan aspek perencanaan diantaranya dengan menghitung kembali jumlah sasaran bayi, menyusun perencanaan sebaik mungkin dan menentukan sumber daya pendukung, sedangkan pada penggerakkan diharapkan kepada bidan desa agar dapat melakukan penggerakkan denggan baik dan melakukan komunikasi yang baik agar terjalin kerjasama lintas program dan sektoral. It was below current minimum services standard in Central Java (90%). A new achievement target for year 2014 is 100%. It means that there was still any gap between the current achievement and the standard. It may be due to not optimal management in implementing an immunization program. This research aimed to analyze management of immunization implementation by village midwives related to achievement of UCI in Batang District. This was observational research with cross-sectional approach. Research instruments used a structured questionnaire and check-list to observe immunization activities. Number of samples was 72 village midwives from 10 health centers in Batang District. Bivariate analysis (Chi-Square test) and multivariate analysis (Logistic Regression) were used to analyze data. The results of this research showed that mostly respondents had good planning (70%), good organizing (87.5%), good actuating (80%), and good controlling (72.5%) at villages which achieve the UCI target. In contrast, at villages which did not achieve the UCI target, respondents had good planning (40.6%), good organizing (65.6%), good actuating (53.1%), good controlling (37.5%), and achieved UCI target (56.6%). The four management aspects of immunization implementation had a significant relationship with achievement of UCI, namely planning (p=0.012), organizing (p=0.027), actuating (p=0.015), and controlling (p=0.003). The result of multivariate analysis indicated that planning (p=0.032) and actuating (p=0.041) influenced achievement of UCI. As a suggestion, midwives who work at villages which do not achieve UCI target need to focus on planning aspects by recalculating a number of babies, rearranging a planning, and determining supporting resources. In addition, they also need to focus on actuating aspects by establishing good communication with other programs and other sectors.
