Abstract

Universitas Diponegoro Fakultas Kesehatan Masyarakat Program Magister Ilmu Kesehatan Masyarakat Konsentrasi Administrasi dan Kebijakan Kesehatan Minat Manajemen Kesehatan Ibu dan Anak 2013 ABSTRAK Oktovina Sophia Rattu Analisis Implementasi Inisiasi Menyusu Dini oleh Bidan di Rumah Sakit Umum Daerah dr. M. Haulussy Ambon 75 halaman + 4 tabel + 3 gambar + 16 lampiran Inisiasi menyusu dini adalah proses membiarkan bayi menyusui sendiri untuk pertamanya saat bayi lahir dan proses ini membutuhkan waktu satu hingga dua jam.Implementasi IMD oleh bidan di RSUD dr.M.Haulussy Ambon hanya sebatas melaksanakan tanpa memperhatikan prosedur/standar IMD yang benar. Tujuan penelitian menjelaskan pelaksanaan IMD yang terdiri dari proses komunikasi, disposisi, ketersediaan sumberdaya dan struktur birokrasi dalam pelaksanaan IMD yang dilakukan bidan di ruang bersalin RSUD dr. M. Haulussy Ambon. Jenis penelitian kualitatif dengan pendekatan cross sectional. Data dikumpulkan melalui wawancara mendalam dengan 5 orang bidan yang menolong persalinan sebagai informan utama dan 8 orang informan triangulasi terdiri 1 kepala ruangan,1 kepala bidang keperawatan, 1 kepala bidang pendidikan dan aktreditasi dan 5 ibu bersalin yang ditolong oleh bidan. Analisis data menggunakan metode analisis isi. Hasil penelitian menunjukkan bahwa semua bidan yang bertugas belum melaksanakan IMD sesuai tatalaksana. Komunikasi pemberi informasi tentang IMD masih antar bidan tidak melalui pimpinan dan waktu penyampaian tidak terjadwal, tenaga bidan untuk menolong persalinan masih kurang, bidan masih kurang memotivasi pasien untuk IMD, belum ada kamar pemulihan. Disposisi bidan belum mendukung IMD, ditunjukkan dengan pemberian susu formula di ruang bayi. SOP IMD yang sudah ada masih tergabung dalam SOP asuhan persalinan normal, SOP baru sedang direvisi. Disimpulkan bahwa IMD oleh bidan belum dilaksanakan sesuai standar IMD. Hal ini dipengaruhi kurangnya komunikasi, tenaga, disposisi/sikap dan SOP yang belum terpisah. Haulussy General Hospital, Ambon 75 pages + 4 tables + 3 figures + 16 enclosures Early breastfeeding initiation (IMD) was a process to let a baby breastfed for the first time after birth. This process needed 1-2 hours. Implementation of IMD by midwives in the district general hospital (RSUD) of dr. M. Haulussy Ambon was limited only activities without following the right IMD standard procedure. Objective of this study was to explain the implementation of IMD by midwives in the delivery room of RSUD dr. M. Haulussy Ambon; the implementation of IMD consisted of communication process, disposition, human resource availability, and bureaucracy structure in the implementation of IMD. This was a qualitative study with cross sectional approach. Data collection was done by conducting in-depth interview to main informants consisted of 5 midwives who assisted delivery process, and to 8 triangulation informants. Triangulation informants consisted of a room chief, a head of nursing unit, a head of education and accreditation unit, and 5 post-delivery mothers who were assisted by midwives. Content analysis was applied in the data analysis. Results of the study showed that all midwives did not implement IMD according to the guidelines. Communication regarding IMD information was still among midwives, and the leader was not included. No schedule was made regarding when the IMD information should be given. The number of midwives who assisted the delivery was insufficient. Midwives were still inadequate in giving motivation regarding IMD to patients; it was shown by giving formula milk in the infant room. Standard operating procedure (SOP) of IMD was still in composite with normal delivery care SOP. A new SOP for IMD was still in revision process. In conclusion, IMD by midwives had not been implemented according to IMD standard. It was influenced by communication insufficiency, human resource, disposition or attitude, and undifferentiated SOP.
