Abstract

Universitas Diponegoro Program Pascasarjana Program Magister Ilmu Kesehatan Masyarakat Konsentrasi Administrasi dan Kebijakan Kesehatan 2012 ABSTRAK Noorhidayah Motivasi Keterlibatan Bidan Praktik Swasta terhadap Program Jampersal di Kota Banjarmasin Provinsi Kalimantan Selatan xv + 101 halaman + 3 tabel + 2 gambar + 12 lampiran Pelaksanaan program Jampersal di Kota Banjarmasin belum berjalan optimal. Pertolongan persalinan oleh non nakes (dukun) meningkat dari 56 pada tahun 2010 menjadi 122 pada tahun 2011. Sosialisasi program Jampersal telah dilakukan oleh Dinas Kesehatan Kota Banjarmasin kepada seluruh bidan. Kepala Dinas Kesehatan telah mengeluarkan instruksi kepada seluruh BPS untuk menjalin kerjasama Jampersal, namun demikian dari 346 BPS yang ada hanya 45 BPS (13%) yang bersedia melakukan perjanjian kerjasama program Jampersal. Rendahnya motivasi BPS untuk melakukan perjanjian kerjasama program Jampersal dipengaruhi oleh faktor intrinsik dan faktor ekstrinsik. Tujuan penelitian ini adalah menjelaskan motivasi keterlibatan BPS terhadap Program Jampersal di Kota Banjarmasin Provinsi Kalimantan Selatan. Jenis penelitian adalah deskriptif kualitatif. Pengumpulan data dilakukan dengan wawancara mendalam pada 8 BPS yang tidak melakukan Perjanjian Kerjasama Program Jampersal sebagai informan utama, Kepala UPTD Jamkesda dan Ketua IBI cabang Kota Banjarmasin sebagai informan triangulasi. Analisis data menggunakan metode analisis isi. Hasil penelitian menunjukkan motivasi BPS untuk melakukan Perjanjian Kerjasama Program Jampersal masih rendah. Dari faktor intrinsik belum ada penghargaan baik dari Dinas Kesehatan maupun IBI kepada BPS yang ikut berpartisipasi dalam Program Jampersal. Dari faktor ekstrinsik tidak ada peraturan yang mewajibkan BPS melakukan Perjanjian Kerjasama Program Jampersal. Tarif Jampersal jauh lebih rendah dari tarif BPS. Pembinaan dan pengarahan kepada BPS oleh Tim Pengelola Jampersal Dinas Kesehatan belum dilakukan. BPS tidak mau melakukan Perjanjian Kerjasama Program Jampersal karena proses pencairan klaim yang lama. Motivasi Keterlibatan BPS terhadap Program Jampersal masih rendah karena faktor intrinsik yaitu tidak ada penghargaan kepada BPS yang ikut berpartisipasi dalam Program Jampersal dan faktor ekstrinsik yaitu belum ada kewajiban, tarif Jampersal rendah dan klaim lama serta belum ada pembinaan dan pengarahan. Berdasarkan hasil penelitian disarankan kepada Dinas Kesehatan dan IBI untuk memperbaiki faktor intrinsik yaitu dengan memberikan penghargaan kepada BPS yang dinilai baik dalam pelaksanaan Jampersal berupa promosi dan dipilih menjadi tenaga medis teladan. Dari faktor ekstrinsik membuat peraturan pendukung dalam pelaksanaan Jampersal, memberikan supervisi dan memperbaiki sistem klaim. Delivery assistance by non health workers (dukun) increased from 56 times in 2010 to 122 times in 2011. Socialization of Jampersal program by Banjarmasin municipality health office to midwives was conducted. Head of municipality health office issued instruction to all private practice midwives (BPS) to perform Jampersal collaboration. However, among 346 BPS, it was only 45 BPS (13%) who were willing to perform Jampersal program collaboration contract. Low motivation of BPS to perform Jampersal program collaboration contract was affected by intrinsic and extrinsic factors. Objective of this study was to explain motivation of BPS involvement in Jampersal program in Banjarmasin city of South Kalimantan Province. This was a descriptive-qualitative study. Data collection was done through in-depth interview to 8 BPS as main informants who did not perform Jampersal program collaboration contract. Triangulation informants were a head of UPTD Jamkesda (district health insurance) and a director of IBI of Banjarmasin city. Content analysis was applied in the data analysis. Results of the study showed that motivation of BPS to perform Jampersal program collaboration contract was low. Intrinsic factor: it was no rewards from both health office and IBI to BPS who participated in Jampersal program. Extrinsic factor: it was no regulation that insisted BPS to perform collaboration contract on Jampersal program; jampersal tariff was far below BPS tariff. No assistance and guidance to BPS from Jampersal management team at district health office was done. BPS did not perform collaboration contract on Jampersal program due to long waiting time for claiming the cost. Motivation of BPS involvement in Jampersal program was still insufficient. This was caused by intrinsic factor: no rewards for BPS who participated in Jampersal program; and by extrinsic factor: no obligation, low jampersal tariff, long waiting time for claiming the cost, and no guidance and assistance. Suggestions for District Health Office and IBI are to improve intrinsic factor by giving rewards to BPS who shows good working performance in the implementation of Jampersal. Rewards can be in the form of rank promotion or to be selected as a good medical worker model. To improve extrinsic factor, it was suggested to formulate regulations to support the implementation of Jampersal; to conduct supervision, and to improve claim system.
