Abstract
Universitas Diponegoro Program Pascasarjana Program Magister Ilmu Kesehatan Masyarakat Konsentrasi Adminstrasi dan Kebijakan Kesehatan 2012 ABSTRAK I Made Raka Efektivitas Pemberantasan Malaria dari Aspek Input di Tingkat Puskesmas Wilayah Kerja Dinas Kesehatan Kota Sorong Provinsi Papua Barat xiii + 99 halaman + 4 tabel + 5 gambar + 13 lampiran Prevalensi kasus malaria di 5 Puskesmas pada wilayah Dinas Kesehatan Kota Sorong sangat bervariasi setiap tahunnya. Untuk tahun 2009 jumlah kasus malaria positif 3372 kasus (20,33‰), meningkat pada tahun 2010 menjadi 4549 kasus (41,84‰). Dinas Kesehatan Kota Sorong sudah mengupayakan berbagai program untuk mengatasi kasus malaria, namun hasilnya belum mengembirakan. Tujuan umum penelitian menjelaskan efektivitas pemberantasan malaria dari aspek input di tingkat Puskesmas wilayah kerja Dinas Kesehatan Kota Sorong. Jenis penelitian kualitatif dengan pendekatan cross sectional. Pengumpulan data dilakukan melalui wawancara mendalam (indepth intervieuw) menggunakan pedoman wawancara. Informan utama adalah Kepala Puskesmas dan Kepala Bidang P2M, sedangkan informan triangulasi adalah tenaga pelaksana malaria yang ada di 5 Puskesmas Kota Sorong. Hasil penelitian menunjukkan bahwa secara kuantitas satu tenaga pelaksana malaria sudah cukup, karena yang bekerja sebenarnya adalah satu tim dalam upaya pemberantasan malaria. Untuk kualitas tenaga pelaksana malaria selama ini dianggap belum memiliki kualifikasi tertentu seperti yang dibutuhkan dan masih perlu mendapat bimbingan. Dana yang dianggarkan untuk pemberantasan malaria masih kurang dan penggelolaanya tidak sesuai dengan unit cost. Tidak ada SOP dan SK Penunjukan yang dibuat oleh Kepala Dinas Kesehatan atau Kepala Puskesmas Kota Sorong, sehingga penempatan tenaga pelaksana malaria hanya berdasarkan penunjukkan saja. Kesimpulan dari penelitian ini menunjukan bahwa pemberantasan malaria belum bejalan dengan baik, terutama dari aspek inputnya. Aspek input yang tersedia belum memenuhi kebutuhan riil dari Puskesmas dalam strategi pemberantasan malaria. Berdasarkan hasil penelitian disarankan untuk dibentuk lagi Kader Malaria Desa di tiap-tiap kelurahan yang ada di wilayah Kota Sorong. Perlu pula ditambahkan tenaga ahli epidemiologi dan entomologi. Bagi petugas pelaksana yang belum pernah mengikuti pelatihan malaria agar segera mungkin untuk dapat diberikan pelatihan. Perlu dilakukan supervisi, monitoring dan evaluasi program yang berkesinambungan. In 2009, the number of positive malaria cases was 3372 (20.33%), it was 4549 (41.84%) cases in 2010. Sorong municipality health office had tried many programs to solve malaria cases; however the result was still inadequate. The study general objective was to explain effectiveness of malaria eradication seen from input aspect in the level of puskesmas in the work area of Sorong municipality health office. This was a qualitative study with cross sectional approach. Data collection was done through in-depth interview guided by an interview guideline. Main informants were a head of puskesmas and a chief of communicable disease control section. Triangulation informants were malaria workers in five puskesmas in Sorong municipality. Results of the study showed that, quantitatively, one malaria worker was sufficient because in reality those who worked in the malaria eradication activities were one team. In regard to the quality or human resource, malaria workers did not fulfill required qualification and they still needed guidance. Funding for malaria eradication was still insufficient, and funding management was not suitable with unit cost. There was no standard operating procedure (SOP). Appointment decrees were issued by the head of municipality health office or by the head of puskesmas Sorong municipality; this showed that allocation of malaria workers was only based on an appointment. In conclusion, malaria eradication program do not run well especially on the input aspect. The provided input aspect has not fulfilled real needs of Puskesmas to implement malaria eradication strategy. Based on the study results, it is suggested to re-form village malaria cadres in every village in Sorong municipality. Epidemiologist and entomologist are needed. Malaria workers who have not attended in malaria training are suggested to be given training as soon as possible. Continual supervision, monitoring, and program evaluation are needed.
