Application of informed choice and informed consent in the North Minahasa was not optimal. This condition was indicated by the decrease of active participants of family planning in the last four years, and the increase of drop out of contraception device users. The study objective was to analyze factors related to the work performance of village midwives in applying informed choice and informed consent during contraception service in the North Minahasa district. This was an observational analytical study with cross sectional approach. Data were collected by conducting interview using questionnaire. Study population was 100 village midwives in the north Minahasa. Fifty respondents were randomly selected from midwives who met the inclusion and exclusion criteria in each puskesmas. Chi-square and logistic regression tests were applied in the data analysis. Results of the study showed that the average age of respondents was 34 years old and the average work length was 11 years; most of respondent’s level of education was D III (60%). Motivation, facilities, perception on workload, and work performance were good. However, knowledge was still insufficient especially on understanding a term of SATUTUJU counseling step, and on how to fill informed consent form. Supervision was still insufficient because it was not done routinely. Factors related to work performance were motivation (p= 0.048), facility (p= 0.001), and perception on supervision (p= 0.001). Factor that jointly related significantly to work performance was perception on supervision (p= 0.001). In conclusion, motivation, facility, and perception on supervision were related to the work performance of village midwives in applying informed choice and informed consent during contraception service. The strongest factor affecting work performance was perception on supervision. Key words : village midwives work performance, family planning counseling service, application of informed choice and informed consent Bibliography : 68 (1993-2012)
