Abstract

Abstrak PT. Indonesia Steel Tube Works (ISTW) adalah perusahaan joint venture antara Indonesia dan Jepang yang memproduksi pipa baja. Dari studi pendahuluan terdapat beberapa jenis cacat untuk periode Oktober-Desember 2011. Akibat dari cacat tersebut perusahaan mengalami kehilangan pendapatan mencapai 83 juta rupiah untuk 3 bulan. Sehingga perlu ada penelitian lebih lanjut untuk mengurangi kerugian peapatan ini. Diantara jenis cacat tersebut terdapat cacat dominan, yang mana cacat welding kurang baik, cacat gores, dan cacat bintik memberikan 80% kontribusi untuk cacat produk. Berdasarkan situtasi yang ada, melakukan root cause analysis adalah hal yang baik guna mencari akar penyebab terjadinya product cacat sehingga kerugian dapat diminimalkan. Dengan metode Fault Tree Analysis dapat terlihat beberapa penyebab yang menyebabkan terjadi produk cacat. Yang mana sebagian adalah dikarenakan oleh kesalahan manusia dalam menjalankan tugasnya. Kemudian dilakukanlah Human Reliabilty Assessment (HRA) untuk mengetahui faktor-fator penyebab terjadinya kesalahan manusia. Metode yang dipakai adalah metode HEART (Human Error Assessment Reduction Technique). Dari analisis yang dilakukan, kemudian disusun rekomendasi pencegahan untuk mencegah terjadinya hal itu, sehingga diharapkan jumlah defect yang terjadi berkurang. Abstract PT.Indonesia Steel Tube Works, a joint venture company between Indonesia and Japan, is a steel tube manufacturing company. From the preliminary study, there were several types of defects for the period October-December 2011. As a result of defects, company suffered a loss of revenue of 83 million rupiahs for 3 months. A research had to be done to reduce this revenue loss. Among the types of defects there were several dominant defects. Not good welding defects, scratch defect, and spots defect gave 80% contribution to a product defects. Given from the situation, doing Root Cause Analysis was good thing to analyse the defects, in order to find the root cause of the defective product, so that losses can be minimized. With Fault Tree Analysis Method, It shows some of the causes that lead to product defects,and some of them were related with Human Error in performing their duties. Then Human Reliabilty Assessment (HRA) was taken to determine factors fator causes of Human Error. The method which is used was the HEART (Human Error Assessment Reduction Technique). After the analysis was done, then drafted recommendations to prevent it from happening in the future, so hopefully it will decrease the number of defects.
