Abstract

Malaria is one of the public health problems in Purworejo District. Cases of malaria from year to year were going down. In 1999, Annual Parasite Incindece (API) was 28.36 per mile. In 2000, API was 44,62 per mile. In 2004, API was 0.77 per mile. API could be minimized but number of villages with High Case Incidence (HCI) status was 27 villages. Program of malaria intervetion in Purworejo District refers to National Strategi by “Gebrak Malaria” which comprises the activities as follows : case fingding and care seeking, eradication of vectors, and surveillance. Activies of case finding and care seeking by Active Case Detection (ACD) method give high contribution (92.01%). These activities have strategic role in the program of malaria eradication. Aim of this research was discribe implementation of malaria finding and treatment by ACD that was done by the village Malaria Worker in the program of malaria eradication ini Purworejo district. This was a qualitative research located at Winong health center, Bener health center, Purworejo health center, and Cangkrep health center. Sample was taken by purposive sampling. Informants were Head of the health center, KUPM, Microscopic Worker, and Village Malaria Worker who worked at the health center. Questioner and ckect List were used to collect data. Result of this research shows that in the input side, there is lack of village malaria worker, KJMD is not available. Completeness of means and tolls to support ACD are enough and can be used well. In the process side, planning has already been done but fund depends on the District Health Office. Implementation of malaria case finding and treatment by village malaria worker has already been done smoothly. Epidemiologi Investigation and Follow Up have not been done in accordance with the guide. The Head of the Health Center has not done supervision and founding optimally because all operational activities are done by KUPM officer. In the output side, coverage of finding towards number of citizen at four health center is 2.98% and it is still under the national target. Coverage of clinical malaria case finding by Passive Case Detection (PCD) is 92.01%. Coverage of malaria positive case finding is 78.18% Follow Up activities for sufferers who get radical treatment and epidemiology investigation of malaria positive case have not been done well. Coverage of medication has been done well. Proportion of P. falciparum in gamet from is high (30.00%). This condition shows that case finding in field is late. The supporting factors for ACD activities are as follows : availability of health facilities, role of other sectors, development of village malaria cadre, and cooperation with district of Purworejo, Magelang, and Kulon Progo (DIY).
