Abstract

Background: The anesthetic technique used in breast surgery at this time has been progressing with the use of regional anesthesia. The single injection of regional anesthesia of paravertebra blocks was developed into new innovations in breast tumor surgery patients. Paravertebra blocks can reduce the otherwise adverse reactions or decrease the patient's response to stress such as plasma cortisol. Aim: This study aims to prove the influence of a single injection of paravertebra blocks which leading to decreased plasma cortisol levels of breast tumor surgery patients. Methods: The study design was observational using a cross sectional analytic study in 10 medical records of patients who underwent surgery with a single injection of paravertebra blocks anesthetic technique that meet the inclusion and exclusion criteria. Then compared the levels of plasma cortisol concentrations before surgery (preoperatively) and after surgery (postoperative). Test statistic used the paired t test. Results: The use of a single injection technique paravertebra block in breast tumor surgical patients who viewed the effect through levels of plasma cortisol concentrations before surgery (preoperatively) which the mean is 258,34 ± 0,91 and after surgery (postoperative) which the mean is 234,01 ± 0,84, has decreased. But the decline did not occur quite significant in all samples. The mean levels of plasma cortisol concentrations were tested using paired t-test statistic test to get the p > 0.05, namely p = 0.163. So that the decreased levels of plasma cortisol concentrations of patients before surgery (preoperatively) and after surgery (postoperative) obtain non-significant results. Conclusion: Single injection of paravertebra blocks caused less significant decreased levels of plasma cortisol concentrations after surgery (postoperative) on breast tumor surgical patients, thus got the non-significant results. Keywords: Single Injection of Paravertebra Block, plasma cortisol levels, breast tumor patients
