Abstract

Background: The technique of spinal anesthesia has side effects such as decreased blood pressure. One way to prevent the decrease in blood pressure is by giving preload. Ringer's lactate is widely used to preload because it easily available and cheap. Aim: To find out the differences in blood pressure after spinal anesthesia with the preload of 20cc/kgBB Ringer's lactate (RL) and without preload(TP). Methods: The study design was experimental with clinical trials phase II randomized single-blind. The sample consisted of 40 patients undergoing spinal anesthesia at the Central Surgery Installation of RSUP Dr. Kariadi Semarang who are included in the study criteria. Patients were divided into 2 groups, group 1 received preload 20cc/kgBB Ringer's lactate and group 2 received no preload. The test is checking blood pressure included systolic blood pressure, diastolic blood pressure and mean arterial press the before and immediately after spinal anesthesia spinal anesthesia every 3 minutes to 15 minutes. If there is a decrease in systolic blood pressure of more than 25% will be given ephedrine 10 mg iv diluted. Also assessed the number of patients and total ephedrine requirements. Hypothesis test is a test used paired t-test. P=0.05 stated value significantly. Results: There were significant differences between the two groups the difference in systolic blood pressure (p=0.005) and mean arterial pressure (p=0.009) which occurred between the pre preload with minute-0 post spinal anesthesia. The mean change in systolic blood pressure pre preload(RL=137.6 18.4; TP=14721.8), minute-0(RL=138.9 19.6; TP=141.5 19.9), minute-3(RL=133.1 19.5; TP=132.223). On the use of ephedrine, RL: 4 samples with an average of 12.5mg of ephedrine and TP: 15 samples with an average of 10mg of ephedrine. Conclusion: There is a significant difference decrease in blood pressure between the provision of preload and without preload Ringer's lactate administration 20cc /kgBB after spinal anesthesia.
