Abstract

Background : Overuse dan irrational use of antibiotics are becoming a worldwide problem, especially in developing countries. Conjunctivitis is one of diseases that potentially resulting an irational use of antibiotics. Pathogenic ambiguity, coupled with the belief that bacterial infections require prescription medication, results in treatment of most presumed cases of infective conjunctivitis with topical ophthalmic antibiotics. The disadvantage of this approach is the possible inappropriate treatment with antibiotics, which raises concerns of antibiotic resistance, cost-effectiveness, and potential increase of complications due to ocular or systemic antibiotic use Methods : This research was a descripive study applied crosssectional design. The material of this research were the medical records of patients with conjunctivitis as the primary diagnosis which had been treated at department of ophthalmology RSUP Dr. Kariadi Semarang in 2010. Quality of antibiotics usage was reviewed by category of Gyssens. The analysis was done descriptively to the appropiateness of indication, the appropiateness of antibiotic choices, the appropiateness of duration of therapy, the appropiateness of dosage and frequencies, dan the appropiateness of route by comapiring the result from medical records with the references. Results : Among 48 medical records that had been analyzed, only 3,7% were classified as category 1 (rational use). There were 43,7% classified as category V (no indication of usage), 96,3% classified as category IV A (there was another antibiotic which was more effective), 96,3% classified as category IV B (there was another antibiotic which was less toxic), and 42,3% classified as category IV C (there was another antibiotic which was less expensive). Conclusion : The rasionalitiy of antibiotics usage in conjunctivitis management was very low. Many antibiotics usage were irrational because there was no indication of usage and wrong antibiotics choice.
