Abstract
Universitas Diponegoro Program Pascasarjana Program Magister Ilmu Kesehatan Masyarakat Konsentrasi Administrasi dan Kebijakan Kesehatan Minat Manajemen Kesehatan Ibu dan Anak 2011 ABSTRAK Ummi Latifah Proses Pengambilan Keputusan Bidan untuk Merujuk Ibu Bersalin dalam Kasus kematian Ibu di Rumah Sakit Umum Daerah Sumbawa Besar Tahun 2011 x + 85 halaman + 3 tabel + 8 gambar + 17 lampiran Angka Kematian Ibu di Sumbawa Besar dari tahun 2007-2010 menunjukkan peningkatan. Kematian ibu sangat berkaitan erat dengan mata rantai rujukan baik ditingkat masyarakat, maupun di fasilitas kesehatan. Mekanisme rujukan belum dilaksanakan dengan optimal sehingga masih sering ditemukan keterlambatan rujukan. Tujuan penelitian ini adalah untuk menjelaskan proses pengambilan keputusan bidan untuk merujuk ibu bersalin ke rumah sakit umum daerah Sumbawa Besar tahun 2011. Penelitian ini menggunakan metode kualitatif dengan pendekatan Studi kasus dan bersifat retrospective. Informan utama penelitian adalah bidan dengan kasus kematian ibu dan informan triangulasi adalah keluarga ibu bersalin, bidan koordinator, kepala ruang bersalin RSUD, ketua IBI Sumbawa Besar. Data dikumpulkan dengan wawancara mendalam, selanjutnya dianalisis dengan tehnik kualitatif. Hasil penelitian ini menemukan bahwa kematian ibu bersalin disebabkan karena keterlambatan dalam proses pengambilan keputusan meliputi keterlambatan mengenali tanda bahaya oleh bidan, keterlambatan setuju merujuk dari keluarga, ketidak tersedianya alat transportasi ketika akan merujuk, jarak yang jauh ke ke rumah sakit rujukan dan sarana jalan yang rusak, dokter spesialis tidak berada di tempat pada saat rujukan tiba di rumah sakit, paritas ke 6 dan faktor usia yang terlalu tua untuk hamil. Dinkes hendaknya mengadakan pelatihan rutin dan terstruktur tentang kegawatdaruratan obstetri terutama pada bidan desa, menggalang partisipasi masyarakat melalui desa siaga sehingga terbentuk dan berjalannya sistem ambulan desa, mengupayakan tersedianya minimal 2 Puskesmas keliling di setiap desa terpencil. Bidan desa, dalam melakukan KIE hendaknya tidak menitikberatkan pada pemberian informasi/penyuluhan tetapi mengabaikan konseling, meningkatkan ketrampilan berkomunikasi dan kerjasama dengan dukun bersalin terutama dalam pendampingan persalinan mengingat persalinan dengan dukun masih cukup tinggi, Menjalin komunikasi dengan masyarakat sehingga dapat terbina hubungan yang baik dan tidak terjadi kesenjangan antara bidan dan masyarakat sekitarnya. Maternal death was related to the chain of referral both in the community level and in the health facility level. Referral mechanism was not optimally done and late referral was frequently found. The objective of this study was to explain midwives decision making process to refer in labor mother to Sumbawa Besar district general hospital in the year 2011. This was a retrospective study using qualitative method and case study approach. The main informants in this study were midwives who experience maternal death during in their works. Triangulation informants were family of in labour mother, coordinator midwives, chief of delivery room of the district general hospital, head of Sumbawa Besar Indonesian Midwifery Association (IBI). Data were collected by conducting in-depth interview and analyzed using qualitative technique. Result of this study showed that in labor maternal death was caused by the delay in decision making process that included delay in identifying danger signs by midwives, delay in having consent for referral from the in labor mother’s family, unavailability of transportation vehicle for referral, long distance from referral hospital and poor road condition, no obstetrician in the hospital when referred in labor mother arrived, the sixth parity and age factor that the mother was too old to get pregnant. It was suggested for district health office to do routine structured training regarding obstetrical emergency to mainly village midwives, to organize community participation via village on alert program to provide and operate village ambulance, to provide at least 2 mobile puskesmas in every remote village. Midwives in implementing KIE were suggested to not only focus on giving education/information but neglecting counseling, to improve communication ability and build collaboration with traditional midwives especially in assisting delivery due to the number of traditional midwives were still high, to build communication with community in order to create good relationships and no gap between midwives and the surrounding community.
