Background: Breast cancer is a common malignancy in women. Evenmore, breast cancer’s incidence in Indonesia is ranked number one, followed by cervix cancer. Spirulina is a microscopic cyanobacterium rich of c-phycocyanin which are able to induce apoptosis. This study aim to find out the effect of Spirulina extract on apoptotic index of T47D cell (ductal breast cancer cell line). Method: This research is an experimental study with Post Test Only Control Group Design. As much as 5 x 104 of T47D cells/1000 μl MK are being divided into ten groups, each consists of two wells. The treatment groups (P1, P2, P3, P4, and P5) were given Spirulina extract dissolved in DMSO with ascending dose, starting from 125 μg/1000 μl MK, 250 μg/1000 μl MK, 500 μg/1000 μl MK, 1000 μg/1000 μl MK, to 2000 μg/1000 μl MK. The control group (K1, K2, K3, K4, and K5) were given DMSO in equal dose to those of the treatment group. Apoptotic index was measured using double staining method. Result: Kruskal-Wallis test showed significans apoptotic index’s differences among groups with p=<0,001 (p<0,05). Mann-Whitney test showed significant differences between K1 and P1 groups (p=0,002), K2 and P2 (p=<0,001), K3 and P3 (p=<0,001), K4 and P4 (P=<0,001), K5 and P5 (p=<0,001), P1 and P2 (p=0,003), P1 and P3 (p=<0,001), P1 and P4 (p=<0,001), P1 and p5 (p=<0,001), P2 and P3 (p=0,007), P2 and P4 (p=<0,001), P2 and P5 (p=<0,001), P3 and P4 (p= <0,001), P3 and P5 (p=<0,001), P4 and P5 (p=0,002). Conclusion: Spirulina extract was proven to increase apoptotic index in T47D cell significantly.
