Abstract

Universitas Diponegoro Program Pascasarjana Program Magister Ilmu Kesehatan Masyarakat Konsentrasi Administrasi Rumah Sakit 2011 ABSTRAK Sihana Analisis Pengaruh Budaya Organisasi Self Discipline terhadap Kepatuhan Perawat Melaksanakan Manajemen Asuhan Keperawatan di Unit Rawat Inap Umum RSUD Kelet Provinsi Jawa Tengah di Jepara 110 halaman + 31 Table + 8 Gambar + 6 Lampiran Kepatuhan perawat dalam melaksanakan manajemen asuhan keperawatan dipengaruhi oleh nilai budaya organisasi Self Discipline, meliputi aspek Acceptance (penerimaan), Will Power (semangat), Hard Work (kerja keras), Productivity (produktivitas) dan Aceptance (ketekunan) yang akan berdampak pada kualitas pelayanan. Manajemen asuhan keperawatan merupakan masalah penting yang harus ditangani pihak Manajemen RSUD Kelet Provinsi Jawa Tengah. Tujuan penelitian adalah diperolehnya gambaran budaya organisasi self discipline (disiplin diri) yang mempengaruhi kepatuhan perawat dalam melaksanakan manajemen asuhan keperawatan di unit rawat inap umum RSUD Kelet Provinsi Jawa Tengah. Penelitian ini merupakan penelitian observasional, dengan pendekatan waktu cross sectional. Populasi penelitian adalah perawat pelaksana di unit rawat inap umum RSUD Kelet Provinsi Jawa Tengah dengan jumlah sampel 48 responden. Analisis statistik yang digunakan analisis bivariat dengan uji chi square dan analisis multivariat dengan regresi logistik. Hasil penelitian menunjukkan bahwa self discipline terkait dengan kepatuhan perawat dalam melaksanakan manajemen asuhan keperawatan pada aspek acceptance rendah 58,3%, aspek will power rendah 58,3%, aspek hard work rendah 22,9%, aspek productivity rendah 31,3%, aspek persistence rendah 37,5%. Hasil analisis bivariat menunjukkan bahwa tidak ada hubungan yang signifikan antara acceptance (nilai p = 0,336), will power (nilai p = 0,810), hard Work (nilai p = 0,860), productivity (nilai p = 1.000) dan persistence (nilai p = 0,359) terhadap kepatuhan perawat melaksanakan manajemen asuhan keperawatan di unit rawat inap umum RSUD Kelet Ptovinsi Jawa Tengah. Analisis multivariat memberikan hasil hanya budaya hard work (kerja keras) yang berpengaruh terhadap kepatuhan perawat melaksanakan manajemen asuhan keperawatan (p=0,547 Exp (β) = 1,618). Saran dalam penelitian ini adalah tingkatkan kepatuhan perawat dalam melaksanakan manajemen asuhan keperawatan pada item pengkajian keperawatan, diagnosis keperawatan dan perencanaan keperawatan, dengan memperkuat sosialisasi budaya organisasi self discipline dengan contoh-contoh kongkrit operasional agar budaya organisasi dipahami tidak hanya sebagai konseptual tetapi juga dapat dipahami sebagai aksi atau perbuatan dalam melaksanakan tugas pokok bidang perawatan. It included acceptance, will power, hard work, productivity and acceptance aspects that would give impact on the service quality. Nurse care management was a significant problem that had to be solved by managerial division of district general hospital (RSUD) of Kelet Central Java province. The objective of this study was to obtain description of self discipline organizational culture that influenced nurse compliance in implementing nursing care management at public inpatient unit of RSUD Kelet Central Java province. This was an observational study with cross sectional approach. Study population was nurses at public inpatient unit of RSUD Kelet Central Java province. The number of samples was 48 respondents. Bivariate analysis using Chi square test and multivariate analysis using logistic regression were applied to analyze data. Results of the study showed that self discipline was related to the nurse compliance in implementing nursing care management. Acceptance aspect was low (58.3%), will power aspect was low (58.3%), hard work aspect was low (22.9%), productivity aspect was low (31.3%) and persistence aspect was low (37.5%). Results of bivariate analysis showed that there was no significant association between nurse compliance in implementing nursing care management at public inpatient unit of RSUD Kelet Central Java province ang acceptance (p = 0.336), will power (p = 0.810), hard work (p =0.860), productivity (p = 1.00), persistence (p = 0.359). Result of multivariate analysis showed only hard work culture that influenced nurse compliance in implementing nursing care management (p = 0.547, Exp (B) = 1.618). Suggestion given in this study was to increase nurse compliance in implementing nursing care management on the item of nursing evaluation, nursing diagnosis and nursing planning. Those would be done by strengthening socialization of self discipline organizational culture by giving operational concrete examples so that organizational culture was understood not only as a conceptual but also as an action or an activity in implementing the main duty in the nursing field.
