Abstract
Background : One of the observation methods to identify high risk cases in the failure of radiotherapy has been used squamous cell carcinoma antigen (SCC antigen) as a tumor marker which is directly known from cervical cancer. The level of SCC has a correlation with activity of cancer cell, so response of radiation therapy can be predicted by measuring SCC antigen level and it can determine prognosis of this patients. Objective : To access the decrease of SCC antigen level in advanced stage epidermoid carcinoma of the uterine cervix after having radiotherapy and the correlation of its with histopathologic radiation response. Settings : This study was done in Obstetric and Gynecology Department and radiotherapy clinic and ward, Dr. Kariadi General Hospital, Medical Faculty of Diponegoro University from July 1, 2005 until March 2006. Material and Methods : The study design was cross-sectional study. There were 45 subjects with advanced stage epidermoid cervical cancer that were measured of SCC antigen level with ELISA before radiation therapy and 3 months after complete radiation therapy and were evaluated histopathologic radiation response by cervical biopsy. The difference of SCC antigen level before and after radiation was analyzed by Mann-Whitney U test. Correlation between SCC antigen and histopathologic radiation response was calculated with X2 test. Results : SCC antigen level was disposed decreased after radiation. From 45 subjects after radiation, there were 2 cases (4.5%) with higher value of SCC antigen level before radiation. SCC antigen level after treatment was identified higher in worse-moderate histopathologic radiation response group than in good response group. The study had cutt off point of SCC antigen level after treatment of 5.7 ng/ml. Conclusion : There is a decrease of SCC antigen level after radiation in most of advanced stage epidermoid cervical carcinoma. SCC antigen level after radiation is > 5.7 ng/ml and it can be used to predict the worse histopathologic radiation response.
