Analysis of Decision and Implementing of Free Treatment Policy (Qualitative Study in District of West Sumbawa, Province of West Nusa Tenggara)

Abstract

A program of free treatment is a policy which is created by Local Government of West Sumbawa District and aimed to improve individual health degree through improving access and quality of basic health services. Sustainability of this program depends on a commitment of the government in providing subsidy for this program. Aim of this research was to analyze the process of decision making, implication possibility, and implementation of the policy at West Sumbawa District. This was a qualitative study using in-depth interview and literature review (secondary documents) to collect data. The informants were a Regent, Parliament Member of Commission I, Head of Regional Development Planning, Health of District Office, Head of Health Center, and Health Workers at Health Center, Sub Health Center, and Village Polyclinic. The factors that are to be a background of free policy are fulfillment of basic needs, improvement of access, services quality, and political factors. Factor demand is not a background of the program. Feasibility study is not performed to arrange and make a decision and determined by executive level. Readiness of funding, infrastructure, provider, and implementer of free treatment is explained by executive level other than legislative level. Most of informants do not really understand financial management. In terms of policy implementation, there are found some complaints related to officialdom problems. Head of health center si the one who knows funding aspects. The implementer at health center level can only obey the procedure of financial management. Policy maker and policy stakeholder propose development of provider and evaluation of free treatment program. Technical implementers propose improvement of their welfare.
