Abstract
BSTRAK Latar belakang : Tanda dini dari hepatotoksik adalah peningkatan enzim-enzim transaminase dalam serum, yaitu alanin aminotransferase (glutamate pyruvate transaminase/SGPT) serta aspartat aminotransferase (glutamate oxaloacetate transaminase/SGOT). Pemberian anestesi propofol atau tiopental dan pembedahan akan mempengaruhi fungsi hepar. Tujuan :Membuktikan adanya perbedaan kadar SGOT dan SGPT antara pemberian tiopental 4,5 mg/kgBB intravena dan propofol 2 mg/kgBB intravena pada induksi anestesi umum. Metode: Penelitian ini merupakan uji klinik fase 2 dilakukan dengan cara quota sampling pada 54 penderita yang menjalani operasi elektif dengan anestesi umum, usia 15-50 tahun, ASA I-II di RS. Dr kariadi Semarang. Penderita dibagi menjadi 2 kelompok : propofol 2 mg/kgBB (I) dan tiopental 4,5 mg/kgBB (II). Spesimen darah dari kedua kelompok diambil sebelum anestesi, menit 10 dan 60. Kadar SGOT dan SGPT diperiksa. Tekanan darah, tekanan arteri rerata, laju jantung dan saturasi O2 dicatat. Uji statistik Mann-Whitney dan independent-t-test. Hasil : Kadar SGPT pada kelompok propofol (24,04±3,45) dan tiopental (27,00±9,76) meningkat pada menit 10 (p = 0,503) dan menurun pada menit 60 (p = 0,066) setelah induksi. Pada kelompok propofol (21,89±5,83) dan tiopental (23,41±6,87) kadar SGOT meningkat pada menit 10 dengan p = 0,567. Pada uji beda antara kedua kelompok, didapatkan perbedaan SGOT pada menit ke 60 dengan p = 0,014. Kesimpulan: Terdapat perbedaan yang bermakna antara pemberian propofol 2 mg/kgBB dan tiopental 4,5 mg/kgBB terhadap kadar SGOT yaitu pada menit ke 60, sedangkan terhadap kadar SGPT tidak bermakna. Kata kunci: tiopental, propofol, SGOT, SGPT ABSTRACT Background: The early sign of hepatotoxic improvement is the increased of serum enzymes transaminase, that is alanin aminotransferase (glutamate pyruvate transaminase/SGPT) and aspartat aminotransferase (glutamate oxaloacetate transaminase/SGOT). Giving tiopental or propofol anesthesia and surgery affect will disturb liver function. Objective: Proving the existence of differences between effects SGOT and SGPT the thiopental 4.5 mg/kg intravena and propofol 2 mg/kg intravena the general anesthesia induction. SGOT and SGPT level review. Blood pressure, average arterial pressure, heart rate and O2 saturation were recorded. Were used as the Mann-Whitney and independent t-test. Results: SGPT levels in propofol (24,04±3,45) and thiopental (27,00±9,76) groups were not increased on the 10th minute (p = 0.503) and decreased on the 60th minute (p = 0.066) after induction. While the SGOT levels in both groups (21,89±5,83 vs 23,41±6,87) were also not increased on the 10th minute (p = 0.567). The SGOT levels in both groups were difference on the 60th minute (p = 0.014). Conclusion: There is significant difference between of the SGOT levels propofol (2 mg/kgbodyweight) and thiopental (4.5 mg/kgbodyweight) on the 60th minute administration.
