Abstract

ABSTRACT Background: Recently NAFLD has been associated with insulin resistance and hypoadiponectinemia. However the study about the relationship of adiponectin level and NAFLD is lacking. Objective: To determine the association between adiponectin level and NAFLD in obese adolescent. To determine discriminant analysis between adiponectin level and HOMA which may predict NAFLD in obese adolescent. Method: A cross-sectional study was conducted in August 2007. The inclusion criteria was obesity adolescents aged 11-14 years. Adiponectin was assessed using ELISA and is defined as hypoadiponectinemia < 2,65 µg/ml. Insulin resistance was obtained by Homeostasis Model Assessment (HOMA > 3,16 mg/dl) which is calculated from blood glucose and fasting insulin. NAFLD was confirmed by abdominal Ultrasonography, which is represented by Bright Liver image. The association between categorial variables was analyzed with Chi-square Test. Result: There were 37 subjects, 26 (70,3%) boys and 11 (29,7%) girls. Twenty subjects (54.1%) had bright liver. Correlation test between adiponectin level and HOMA was negatif (r = -0,503 , p: 0,001). There were no significant association between hypoadiponectinemia and bright liver (p: 0,350). Discriminant analysis between adiponectin level and HOMA > -0,25 may predict NAFLD in obese adolescent. Conclusion: Hypoadiponectinemia is not associated with NAFLD in obese adolescent. Discriminant analysis between adiponectin level and HOMA > -0,25 may predict NAFLD in obese adolescent. Keywords: adiponectin, NAFLD, obesity, adolescent ABSTRAK Latar Belakang: Penyakit perlemakan hati non alkohol berhubungan dengan resistensi insulin dan hipoadiponektin. Adapun penelitian mengenai hubungan kadar adiponektin dan penyakit perlemakan hati non alkohol masih kurang. Tujuan: Membuktikan adanya hubungan kadar adiponektin dengan penyakit perlemakan hati non alkohol pada remaja obesitas. Membuktikan persamaan kadar adiponektin dan HOMA untuk mengetahui keberadaan penyakit perlemakan hati non alkohol pada remaja obesitas. Metode: Penelitian belah lintang yang dilakukan pada bulan Agustus 2007. Dengan kriteria inklusi remaja obesitas usia 11-14 tahun. Kadar adiponektin diperiksa menggunakan ELISA. Hipoadiponektin apabila kadar adiponektin < 2,65 µg/ml. Resistensi insulin didapatkan dari nilai Homeostasis Model Assessment (HOMA > 3,16 mg/dl) yang diukur dari kadar glukosa puasa dan kadar insulin. Penyakit perlemakan hati non alkohol diketahui melalui ultrasonografi abdomen dengan hasil Bright Liver. Hubungan dari variabel kategorikal dianalisa dengan Chi-square Test. Hasil: Dari 37 subyek, laki – laki 26 subyek (70,3%) dan perempuan 11 subyek (29,7%). Penyakit perlemakan hati non alkohol 20 subyek (54,1%). Uji korelasi antara kadar adiponektin dengan HOMA (r = -0,503 , p: 0,001). Uji Kai-Kuadrat antara hipoadiponektin dengan penyakit perlemakan hati non alkohol (p: 0,350). Persamaan kadar adiponektin dan HOMA > -0,25 dapat mengetahui penyakit perlemakan hati non alkohol pada remaja obesitas. Simpulan: Hipoadiponektin tidak berhubungan dengan penyakit perlemakan hati non alkohol pada remaja obesitas. Persamaan kadar adiponektin dan HOMA > -0,25 dapat mengetahui penyakit perlemakan hati non alkohol pada remaja obesitas.
