Backgrounds : Leptospirosis is a zoonosis that caused by infection with pathogenic Leptospira species. Leptospirosis is now identified as one of the re-emerging infectious diseases and have a high incidence and exposure risk with a varies clinical manifestation. Clinical manifestations of human leptospirosis may become a potentially fatal disease. The reported mortality rates of severe leptospirosis range between 5% and 40% or even higher. What are the prognostic factors associated with mortality patients with severe Leptospirosis. Objective : The purpose of this research is to know the prognostic factors on admission of patients with severe leptospirosis. Methods: This was a cross sectional study conducted by the Semarang-Amsterdam Leptospirosis Studies (SEALS) with confirmed diagnosis of severe leptospirosis (positive test of Lepto Dri-Dot and MAT ). Result : A total of 106 patients included in the study consited 0f 54 males and 52 females, with mean age 56 years. Multivariate logistic regression demonstrated that 3 factors on admission were independently associated with mortality : age > 60 years (p = 0,037 ; OR Adjusted 0,08 ; 95% CI 0,009 – 0,869), mialgia (p = 0,026 ; OR Adjusted 13,89 ; 95% CI 1,375 – 140,37), and calve pain extremities (p = 0,003 ; OR Adjusted 0,04 ; 95 % CI 0,006 – 0,333). Conclusions : Age > 60 years , mialgia, and calve pain extremities, were prognostic factors associated with mortality patients with severe leptospirosis and the probability of the test is 46%.
