Abstract
Background : Iopamidol, a low osmolarity contrast media (LOCM) has better track record of nephrocity among the others, but it still role to develop an adverse effect. It could cause a nephrotoxicity which induce an acute tubular necrosis. L-Arginin as a natural precursor of nitric oxide (NO) might prevent vascular vasoconstriction that cause necrosis. Objectives: To know the effect of high dose iopamidol intravena, the influence of L-Arginin before iopamidol was injected. Methods: That was a post test only control group design experimental study using 35 three months Sprague –Dawley rats which were randomly allocated into seven groups. There were K- group, which were not given any treatments, P1; P2; P3 group given 1,8 ml iopamidoland histological examination were done to assess acute tubular necrosis with HE staining observed on 24, 48, and 72 hour accordingly. P4; P5; P6 group given 8,4% (W/V) of L-Arginin personde twice a day, 5 ml each for 7 days administration before given 1,8 ml iopamidol for histological examination were done to assess acute tubular necrosis with HE staining observed on 24, 48, and 72. Samples weregiven free access to tap water and standar rat chow. Results : The average acute tubular necrosis is K- (9,0¬¬ + 0,7), P1 (30,0 + 12,4), P2 (35,0 + 21,6), P3 (49,0 + 8,0), P4 (29,0 + 21,8), P5 (34,0 + 23,6), P6 (20,0 + 3,9). There is significantly difference with ANOVA test, so then we use post hoc Tamhane test, the result is acute tubular necrosis increased following iopamidol administration in 72 hour (P3) p=0,007 and L-Arginin reduced acute tubular necrosis on group P6 (p=0,008) Conclusion : Iopamidol induce acute tubular necrosis significantly and L-Arginin reduced acute tubular necrosis on group P6 (p=0,008) Keywords : iopamidol, serum creatinin, acute tubular necrosis
