Abstract

The success of Village Alert Development Program can be shown by the levels of stratification reached. Before information system is applied, the stratification for village alert program could not operated as there were no standard report form, no evaluation form, and the difficulty in calculating the strata. Thus, the evaluation of village alert program has not conducted yet since there are no available valid data. The aim of this research is to develop information system of the village alert stratification which supports the evaluation program of Village Alert Development Programs in Central Java Health Office. Based on the results of the observation and interview, it was shown that the application of village alert stratification faced problems from input, process and output phases. The need of data and information could not be fulfilled, the stratification evaluation forms were not available and Data Base Management Information System had not been implemented. From this study it was concluded that by development of Information System on Stratification of Village Alert, the essential information and data of the development village alert evaluation program could be accomplished. The evaluation of Information system of village alert showed the increase in information quality from baseline (considered average 1.84) to after information system development (considered average 4.17). Furthermore, this system can be used for data entry and information searching. This system is easy to operate, simply to understand, safety for data and satisfied users.
