Abstract

Background: Diffuse injury is a form of head injuries most often occur. Raised intracranial pressure (RICP) is a major cause of death in severe head injury and major complications in patients with moderate and mild head injury. RICP based on CT-scan images can be indicated from the loss of the groove, compression of the ventricles, and obliteration of the sisterna. Inflammatory response and increased epinephrine and cortisol on head injury can increase the number of peripheral blood leukocytes. This study aimed to investigate the correlation between intracranial pressure based on the CT-Scan and the number peripheral blood leukocytes in diffuse injury patients. Methods: This study design is cross-sectional using diffuse injury patients data as sample. A total of 84 samples that meet the criteria in Neurosurgery Department of Dr. Kariadi Hospital during January 2006-July 2010 were divided into two groups. Diffuse injury patients grade I and II are grouped into patients without RICP. Diffuse injury patients grade III and IV are grouped into patients with RICP. All samples underwent a total leucocyte count on the first day. The data described in the form of tables and Spearman test with SPSS for Windows 15.0. Results: The mean number of peripheral blood leukocytes in diffuse injury patients is 16 282,14/mm3. The mean number of peripheral blood leukocytes in diffuse injury patients without RICP is 15 080,30/mm3 and in diffuse injury patients with RICP is 20 688.89/mm3. Spearman test results between intracranial pressure based on the CT-Scan and the number of leukocytes have the value p=0.0001 and r=0.458. Conclusion: Total peripheral blood leukocytes in patients with diffuse injury is higher than normal number of peripheral blood leukocytes. There is a significant correlation with positive direction and moderate power between intracranial pressure based on the CT-Scan and the number of peripheral blood leukocytes in diffuse injury patients.
