Abstract

Background : The high levels of bacterial resistance in the ICU is a matter that leads to the preliminary setting up and monitoring the use of antibiotics in the ICU. World Health Organization has recommended to the medical education institutions to assess the rationality of the use of antibiotics. Objective : Assessing the level of antibiotic use based on Defined Daily Doses/100 patients and Gyssens’ criteria are viewed from quantity and quality aspects in the ICU, Dr. Kariadi the period from August to December 2008. Methods : This study using 40 samples taken by simple random sampling method. Assessment of the quantity of antibiotic use was done by two ways, first by calculating the maximum dose and minimum dose, and the second by counting DDD/100 patients. Antibiotics are used will be assessed for compliance with the formulary of Dr. Kariadi hospital. The study of antibiotic use in quality is displayed according to Gyssens category. Results : That was found a discrepancy between the maximum dose with minimal doses of 89,47%. Ceftriaxone is the most widely used drug that is equal to 62,2 DDD/100 patients. Based on the type of therapy of 40 medical records consists of 57,9% ADE; 38,2% ADET, and 3,9% ADP. By category Gyssens 19,7%, including category I (rational); 1,3% category IIA (not rational because it is not appropriate dose); 2,6% Gyssens category IIB (interval irrational because of inappropriate antibiotic use); 30,3% Gyssens category IIIA (irrational because of prolonged antibiotic use); 5,3% Gyssens category IIIB (irrational because of antibiotic given too short); 36,8% Gyssens category IVA (irrational because there are other antibiotics that more effective) and 3,9% category Gyssens IVD (irrational because there are other antibiotics that spectrum is narrower). Conclusion : There were inappropriate antibiotic use including both of quantity and quality. Key word : Rationality of antibiotic use, quantity, quality, DDD, Gyssens category.
