Abstract

Background: Prostate carcinoma is the second most common tract urinary malignancy after bladder carcinoma. Prostate Specific Antigen serologic examination is very useful for early detection, and for monitoring postoperative in prostate carcinoma. This aim research is to determine the accuracy of PSA in variation of levels and also to compare which level of PSA is currently have more significant result. Method: Patient with prostate mass were referred by the sub department of urologic surgery dr. Kariadi general hospital Semarang and underwent PSA serologic examination between 2000 until 2006. Patient who have had postoperative prostate surgery, radiation therapy, chemotherapy were excluded. The sensitivity and specificity were determine by using 2x2 table. Results: PSA Examination was detected in 100 subjects with prostate mass. The PSA cutpoint of 4 ng/ml had a sensitivity of 79% and a specificity of 19%. PSA testing was most sensitive but least specific in men. Age-specific reference ranges improved specificity in older men but decreased sensitivity. Increasing the PSA cut point to 10 ng/ml resulted in a sensitivity of 66%, a specificity of 43%. Conclusions: PSA testing had fair discriminating power for detecting prostate cancer in Dr. Kariadi general hospital Semarang. The PSA cut point of 4 ng/ml was sensitive but relatively non-specific for cancer. Using age-specific reference ranges and a PSA cutpoint above 4 ng/ml decreased test sensitivity and increased specificity. Key words: Prostate Cancer , Prostate Specific Antigen
