Abstract
Background: Bidara (Andrographis paniculata) leaf extract has a suppression effect in multiplication of malaria parasite. This extract can increase natural immunity (macrophage activity) and acquired immunity (antibody levels). Hypoglycemia is a complication of malaria, while Bidara leaf extract can decrease blood glucose level. The objective of this experiment was to find out the effect of parasitemic levels on blood glucose level during the infection of Plasmodium berghei ANKA (P. berghei ANKA) in Balb/c mice treated with Bidara leaf extract. Subjects and Method: It was an experimental study, used The Post-Test Only Control Group Design. Ninety Balb/c mice used and divided into 2 groups: treated and control. The treated group receivad Bidara leaf extract 16,608 mg was given per oral per day, which began 11 days pre-infection until 8 days post-infection, while the control group received CMC-Na. The blood glucose concentrations was measured on the third, sixth, and eighth day of post inoculation. The result was processed with Kruskall-Wallis test and subsequently with Mann-Whitney if the value was significant. Result: The blood glucose level on control group was significantly lower than treated group on sixth day of infection. However, correlation between parasitemic levels and blood glucose levels was not observed either in control or treated group. The blood glucose level on treated group was significanty lower than normal value only on day eight of infection, while in the control group this was occured on day 6 and 8 infection. The kinetic of blood glucose level demonstrated that a significant decrease glucose concentration occurred on day six in control group and on day eight in treated group. Conclusion: The parasite load might only play in minor role in decreasing blood glucose concentration in P. berghei ANKA infected Balb/c mice. Bidara leaf extract has protective effect in maintaining the normal blood glucose level of Balb/c mice especially on day six, but not on day 8 of P. berghei ANKA infection. Therefore, the protective effect of Bidara leaf extract is not complete, and the extract can only slowdown the decrease blood glucose levels. Latar belakang: Ekstrak daun Bidara (Andrographis paniculata)) terbukti dapat menghambat perkembangan Plasmodium berghei ANKA (P. berghei ANKA) Ekstrak inijuga dapat meningkatkan respon kekebalan alamiah (makrofag) dan kekebalan didapat (antibodi). Hipoglikemia adalah salah satu komplikasi pada infeksi Plasmodium. Sementara ekstrak daun Bidara mempunyai pengaruh menurunkan kadar glukosa darah. Tujuan penelitian ini untuk mengetahui pengaruh tingkat parasitemi terhadap kadar glukosa darah selama infeksi P. berghei ANKA pada mencit Baffi/c yang diberi ekstrak daun Bidara. Bahan dan Metoda: Desain penelitian ini adalah post test only control group. Sampel penelitian menggunakan 90 ekor mencit dewasa, dibagi menjadi 2 kelompok yaitu kelompok perlakuan dan kelompok kontrol. Kelompok perlakuan mendapat preparat ekstrak daun Bidara sebanyak 16,608 ml per had per oral, mulai I I had pre infeksi sampai 8 hari post infeksi. Sedangkan pada kelompok kontrol diberikan CMC-Na 0,5% Kadar glukosa darah diamati pada had ke-3, ke-6, dan ke-8 post infeksi. Uji beda antar kelompok (perlakuan, kontrol, normal) dan uji beda antar had menggunakan uji Kruskal Wallis jika signifikan dilanjutkan uji Mann-Whitney. Hash!: Kadar glukosa darah pada kelompok kontrol lebih rendah secara bermakna dibanding kelompok perlakuan pada hari ke-6 post infeksi. Korelasi antara tingkat parasitemia dengan kadar glukosa darah tidak didapatkan baik pada kelompok kontrol maupun perlakuan selama perjalanan malaria. Kadar glukosa darah pada kelompok secara bermakna dibawah kadar normal pada had ke-6 dan hari ke-8 infeksi, sementara hanya pada had ke-8 pada kelompok perlakuan. Kinetik kadar glukosa darah mencit menunjukan penurunan bermakna sejak hari ke-6 infeksi pada kelompok kontrol dan hanya pada had ke-8 infeksi pada kelompok perlakuan. Kesimpulan: Tingkat parasitemia mungkin hanya mempunyai pengaruh kecil pada kadar glukosa darah selama infeksi P. berghei ANKA pada mencit Balb/c. Ekstrak daun Bidara berpengaruh mempertahankan kadar normal glukosa darah mencit Balb/c khususnya had ke-6 infeksi. Efek proteksi ini tidak sempurna karena ekstrak daun Bidara hanya memperlambat terjadinya penurunan kadar gula darah.
