Abstract

The Effect of Andrographis paniculata (Sambiloto) Extract to the Glutamic Oxaloacetic Transaminase Serum Level in Wistar Which were Induced by Paracetamol Anggraeni Sagita*, M. Masjhoer** Background: The level of Serum Glutamic Oxaloacetic Transaminase (SGOT) could be elevated higher than normal level due to disturbances in cell membrane integrity on hepatocelluler necrosis caused by virus and hepatoxic drugs like paracetamol. Andrographis paniculata (A. paniculata) can protect liver damage caused by paracetamol through the compound contained inside, andrographolide. This compound could decrease lipid peroxidation, increase number of glutathione, and increase antioxidant enzymes activities. The aim of this study was finding the ability of A. paniculata extract to decrease the level of SGOT caused by paracetamol. Subject and method: The study was a true experimental study with the post test only control group design. The samples were 20 male Wistar rats with 3-4 months old and then randomly divided into 4 groups. Group 1 (negative control) received standard diet only, group 2 (positive control) received single dose of paracetamol 1350 mg/kgBW at day 8, group 3 (Treatment 1) received 500 mg/kgBW/day A. paniculata extract for first 7 days and received single dose of paracetamol 1350 mg/kgBW at day 8, and group 4 (Treatment 2) received single dose of paracetamol 1350 mg/kgBW at day 8 and received 500 mg/kgBW/day A. paniculata extract on day 8-day 10. All groups were terminated at day 10 after blood samples were taken from retroorbita for measurement of the SGOT level. Results: The results of Anova test showed that there were significant differences on SGOT level (p=0,000). Post-hoc analysis using Bonferonni test showed significant differences between each groups on SGOT level (p=0,000). Conclusion: A. paniculata extract could decrease SGOT level as indicator of liver damage caused by paracetamol in Wistar rats. Keywords: Andrographis paniculata, SGOT, paracetamol. Masjhoer** Latar belakang: Pada nekrosis hepatoselluler yang disebabkan oleh virus dan obat-obat hepatotoksik seperti parasetamol, kadar Serum Glutamat Oksaloasetat (SGOT) dapat meningkat lebih dari nilai normal karena integritas membran sel terganggu. Andrographis paniculata (A. paniculata) dapat melindungi hati akibat toksisitas parasetamol melalui senyawa yang terkandung didalamnya, yaitu andrographolide. Senyawa ini dapat menurunkan peroksidasi lipid, meningkatkan jumlah glutation, dan meningkatkan aktivitas enzim antioksidan. Penelitian ini bertujuan untuk mengetahui kemampuan ekstrak A. Paniculata untuk menurunkan kadar SGOT tikus Wistar yang diberi parasetamol. Bahan dan Metode: Penelitian ini adalah eksperimental murni dengan desain post test only control group. Sampel penelitian adalah 20 ekor tikus Wistar jantan, umur 3-4 bulan, dan secara random dibagi menjadi 4 kelompok. Kelompok 1 (kontrol negatif) hanya mendapatkan pakan standar, kelompok 2 (kontrol positif) yang mendapat 1350 mg/kgBB parasetamol dosis tunggal pada hari ke-8, kelompok 3 (Perlakuan 1) yang mendapat ekstrak A. paniculata 500 mg/kgBB/hari 7 hari pertama dan mendapat 1350 mg/kgBB parasetamol dosis tunggal pada hari ke-8, dan kelompok 4 (Perlakuan 2) yang mendapat 1350 mg/kgBB parasetamol dosis tunggal pada hari ke-8 dan diberikan A. paniculata 500 mg/kgBB/hari dari hari ke-8 sampai hari ke-10. Pada hari ke-10 semua kelompok diterminasi setelah dilakukan pengambilan sampel darah secara retroorbita untuk pemeriksaan kadar SGOT. Hasil: Uji Anova menunjukkan bahwa terdapat perbedaan kadar SGOT yang bermakna (p=0,000). Pada analisis post-hoc yang memakai uji Bonferonni didapatkan perbedaan kadar SGOT yang bermakna antara masing-masing kelompok(p=0,000). Kesimpulan: A. paniculata dapat menurunkan kadar SGOT tikus Wistar yang merupakan indikator kerusakan hati setelah pemberian parasetamol. Kata kunci: Andrographis paniculata, SGOT, parasetamol.
