PERBEDAAN PENGARUH PEMBERIAN ANESTESI SPINAL DENGAN ANESTESI UMUM TERHADAP KADAR GULA DARAH

Abstract

THE DIFFERENT OF THE EFECT BETWEEN SPINAL ANESTHESIA AND GENERAL ANESTHESIA ON BLOOD GLUCOSE LEVEL BEFORE AND AFTER ANESTHESIA INDUCTION Asri Suryaniati1), Heru Dwi Jatmiko2) ABSTRACT Background : Anesthesia and operation procedure could evoke stress respons,that increase the blood glucose concentration and make a hyperglicemia condition. It is important to find a safe anesthesia procedure, whether spinal or general anesthesia procedure in according to minimize the increase of blood glucose concentration. Objective : To determine blood glucose concentration rate between patients with spinal and general anesthesia procedure. Methods : Analitic study was done with cross sectional included 40 patients which devided into 2 groups using consecutive sampling . Group I was group with spinal anesthesia procedure using Lidodex 100mg and followed by ephedrine 10mg iv soon after the spinal anesthesia drug given. Group II was group with rapid general anesthesia procedure using atracurium 5mg, propofol 2-2,5mg/kg, succynilcholine 1mg/kg followed by more atracurium 15mg, and maintained by N2O : O2 = 50% and ½-1 MAC of Isoflurane. After delivery, tramadol 2mg/kg was given. The measurement of blood glucose concentration was taken from arteriole blood for 2 times (preanesthesia and postanesthesia) by Blood Glucose Test Meter GlucoDr device. Statistic test were performed by Kolmogorov-Smirnov Test, Mann-Whitney Test. Results : There were decreases in blood glucose concentration of post anesthesia compared with preanesthesia condition in both groups. The decrease of blood glucose concentration rate was bigger in Group I (3,90%) than in Group II (2,75%). Conclusion : There is no differences in the changes in blood glucose concentration between the spinal and general anesthesia procedure. Oleh karenanya perlu dicari cara anestesi yang paling sedikit menaikkan kadar gula darah, dengan spinal ataukah anestesi umum. Tujuan : Mengetahui perbedaan rata-rata kadar gula darah antara pasien yang mendapat anestesi spinal dengan pasien yang mendapat anestesi umum. Metode : Penelitian ini merupakan penelitian analitik retrospektif dengan rancangan cross sectional. Pada 40 pasien yang dibagi menjadi dua kelompok. Pemilihan sampel menggunakan consecutive sampling. Kelompok I memperoleh anestesi spinal dengan menggunakan obat lidodex 100 mg yang dilanjutkan pemberian ephedrin 10 mg i.v. segera setelah obat spinal masuk. Kelompok II memperoleh anestesi umum dengan induksi cepat, obat yang digunakan atracurium 5 mg, propofol 2-2,5 mg/kgbb, succinylcholin 1 mg/kgbb dan atracurium lagi 15 mg. Rumatan menggunakan N2O : O2 = 50% dan isoflurane ½ - 1 MAC. Setelah bayi lahir diberikan tramadol 2mg/kgbb. Pengukuran kadar gula darah menggunakan darah arteriol yang diukur 2 kali (pre anestesi danpost anestesi) dengan alat Blood Glucose Test Meter GlucoDr. Uji statistik dilakukan dengan Koimogorov-Smirnov Test untuk mengetahui normalitas distribusi sampel, Mann-Whitney Test. Hasil : Terjadi penurunan kadar gula darah post anestesi dibandingkan dengan kadar gula darah pre anestesi pada kedua kelompok dengan kelompok I penurunannya lebih besar sekitar 3,90 gr%, sedang kelompok II hanya sekitar 2,75 gr%. Kesimpulan : Tidak ada perbedaan yang bermakna cara anestesi spinal dengan anestesi umum terhadap kadar gula darah.
