Abstract
Latar belakang : Cedera kepala tertutup (Closed Head Injury) mempunyai insidensi yang masih sangat tinggi.Adanya massa intrakranial menyebabkan terjadinya kenaikan Tekanan intrakranial (TIK). Kenaikan TIK ini dapat mengakibatkan pengurangan suplai darah ke otak yang akan mengakibatkan serangkaian iskemia sehingga akan terjadi kerusakan otak. Monitoring terjadinya iskemia otak sangat diperlukan, GFAP (=Glial Fibrilary Acidic Protein) merupakan actual marker pada kerusakan sel astroglia. Belum didapatkan suatu penelitian tentang hubungan antara jumlah volume hematoma epidural (EDH) dengan GFAP plasma darah Tujuan : Membuktikan adanya hubungan antara peningkatan volume hematoma epidural (EDH) dengan kenaikan titer GFAP plasma darah . Metoda : Penelitian ini merupakan penelitian observasional klinis dengan desain cross sectional. Dilakukan penelitian pada 14 pasien cedera kepala yang pada hasil pemeriksaan CT-Scan didapatkan lesi berupa hematom epidural (EDH) tanpa kelainan lain. Diukur volume EDH dari CT-Scan, kemudian diambil serum darah vena perifer cephalica dan diperiksa titer GFAP dengan metoda ELISA. Analisa statistik dilakukan dengan uji korelasi Pearson antara variabel volume EDH dengan titer GFAP plasma . Hasil : Pada uji korelasi Pearson didapatkan korelasi antara peningkatan volume EDH dengan peningkatan titer GFAP plasma (p=0,000), dengan koefisien korelasi 0,992. Simpulan : Ada hubungan positif kuat antara peningkatan volume EDH dengan peningkatan titer GFAP plasma Background : High incidency of closed head injury still occure. Intracranial mass can cause increase of Intracranial Pressure (ICP). The impact of high ICP is decrease in cerebral perfusion pressure that cause brain tissue ischaemia. Due to this condition, the evaluation of brain tissue ischaemia is very essential, GFAP (=Glial Fibriliary Acidic Protein) is the most actual marker of astroglia cell damage. There is not study to prove the correlation between the volume of Epidural Hematoma (EDH) and increase of GFAP blood plasma consentration yet. Objective : To prove the correlation between the volume of Epidural Hematoma (EDH) and increase of GFAP blood plasma consentration. Material and Method : The research was done as a clinical observation with cross sectional design. Fourteen brain injury patients who were CT’s diagnosed EDH without any CT’s abnormality , then performed examination about the EDH volume and GFAP’s blood plasma consentration with ELISA method. Statisticaly analysis had been done with Pearson’s correlation study. Result : Pearson’s correlation study showed a correlation between the increased of EDH volume and GFAP’s plasma consentration with p=0,001, and correlation coefficient 0.992. Conclusion : There is a strong positive correlation between the of EDH volume and GFAP ‘s plasma concentration.
