Abstract
Obedience is an attitude to obey a decided regulation without expecting a reward. Besides obeying a regulation, people must obey to a leader’s instruction, usage of time, a job responsibility, and their skill. Low obedience of doctors in writing a prescription based on formula was a main problem which must be overcome by the management of Prof. Dr. W. Z. Johannes Public Hospital in Kupang. The objective of this research was to find out the internal and external factors that influence the obedience of doctors in writing a prescription for patients at the Outpatient Unit based on formula at Prof. Dr. W. Z. Johannes Public Hospital. This was an observational research with cross-sectional approach. Population was all doctors who had tasks and functions as providers of medical services at the Outpatient Unit of Prof. Dr. W. Z. Johannes Public Hospital. Data were analyzed using the methods of bivariate analysis (Chi Square Test) and multivariate analysis (Logistic Regression with Enter method). The result of a descriptive analysis showed that the percentage of the respondents who were obedient was 38.6%. The percentage of respondents who had an important knowledge was 68.18%, an important confidence was 47.73%, a good attitude was 54.55%, a good reward system was 38.64%, a good information system was 79.55%, and a good punishment system was 36.36%. The result of the bivariate analysis showed that the variables of confidence (p=0.036), attitude (p=0.045), reward system (p=0.001), and punishment system (p=0.033) had a significant relationship with the obedience of the doctors in writing a prescription based on formula of the hospital. Furthermore, the result of multivariate analysis showed that the variables of confidence (p=0.570; Exp(B)=2.290) and reward system (p=0.000; Exp(B)=352.192) together influenced the obedience of the doctors in writing a prescription based on formula at Prof. Dr. W. Z. Johannes Public Hospital. As a suggestion, the hospital management should accommodate a doctor’s needs at the Outpatient Unit in writing a prescription of patent medicines and improve a reward system in terms of writing a prescription for patients at the Outpatient Unit. Kepatuhan adalah sikap mentaati peraturan dan ketentuan yang telah ditetapkan tanpa pamrih. Di samping mengandung arti taat dan patuh pada peraturan, juga kepada perintah pimpinan, sikap perhatian dan kontrol yang kuat terhadap penggunaan waktu, sikap tanggung jawab atas tugas yang diamanatkan kepadanya, atau sikap kesungguhan terhadap bidang keahlian yang ditekuninya. Rendahnya tingkat kepatuhan dokter dalam menulis resep berdasarkan formularium rumah sakit merupakan permasalahan penting yang harus segera ditangani oleh manajemen RSUD Prof. Dr. W. Z. Johannes Kupang. Tujuan penelitian ini adalah untuk mengetahui faktor-faktor internal dan eksternal yang mempengaruhi kepatuhan dokter dalam menulis resep pasien rawat jalan sesuai dengan formularium RSUD Prof. Dr. W. Z. Johannes Penelitian ini merupakan penelitian observasional, dengan pendekatan rancangan penelitian cross sectional. Populasi penelitian adalah seluruh dokter yang melaksanakan tugas dan fungsinya sebagai pemberi pelayanan medik di Instalasi Rawat Jalan RSUD Prof. Dr. W. Z. Johannes Kupang. Analisis statistik yang digunakan analisis bivariat dengan uji chi square dan analisis multivariat dengan uji regresi logistik metode enter. Hasil analisis deskriptif, kepatuhan dokter (38,6%), pengetahuan penting (68,18%), keyakinan penting (47,73%), sikap baik (54,55%), sistem penghargaan baik (38,64%), sistem informasi baik (79,55), dan sistem sanksi baik (36,36%). Hasil analisis bivariat menunjukkan bahwa ada hubungan yang signifikan antara keyakinan (p=0,036, p<0,05), sikap (p=0,045, p<0,05), sistem penghargaan (p=0,001, p<0,05) dan sistem sanksi (p=0,033, p<0,05) terhadap kepatuhan dokter menulis resep berdasarkan formularium rumah sakit). Hasil analisis multivariat menunjukkan adanya pengaruh bersama-sama antara keyakinan (p=0,570, Exp(B)=2,290), dan sistem penghargaan (p=0,000, Exp(B)=352,192) terhadap kepatuhan dokter menulis resep berdasarkan formularium RSUD Prof. Dr. W. Z. Johannes Kupang. Saran yang direkomendasikan dalam penelitian ini adalah mengakomodir kebutuhan dokter di Instalasi Rawat Jalan dalam menulis resep obat paten, serta memperbaiki reward system terkait penulisan resep pasien rawat jalan umum.
