Abstract
Latar belakang : Kontrol terhadap nyeri akan meningkatkan respon imun dalam penyembuhan luka sehingga penyembuhan luka lebih baik. Salah satu cara mengontrol nyeri dapat menggunakan infiltrasi anastetik lokal disekitar luka. Levobupivakain merupakan obat anestetik lokal dengan durasi panjang. Sistem imun yang memegang peran penting dalam penyembuhan luka salah satunya adalah CD8+. CD8+ merupakan downregulator of wound healing. Rasio CD4+/CD8+ juga menunjukan proses penyembuhan luka. Dalam penelitian ini diharapkan gambaran CD8+ dan rasio CD4+/CD8+ dapat mencerminkan penyembuhan luka akibat infiltrasi levobupivakain. Tujuan : Menganalisis hubungan pemberian infiltrasi anestetik lokal levobupivakain di sekitar luka dengan skor histologi CD8+ dan rasio CD4+/CD8+. Metode : Dilakukan penelitian eksperimental laboratorik dengan disain “Randomized post test only control group design”, pada 10 ekor tikus Wistar. Kelompok penelitian dibagi menjadi dua kelompok secara acak masing masing 5 ekor. Kelompok 1, tikus yang dilakukan insisi 2 cm, disuntik tiap 8 jam selama 24 jam tanpa infiltrasi levobupivakain. Kelompok 2, tikus yang dilakukan insisi 2 cm, infiltrasi levobupivakain tiap 8 jam selama 24 jam. Kandungan CD8+ pada sekitar luka insisi dinilai dengan skor histologi dari preparat yang dilakukan pengecatan secara imunohistokimia dengan menggunakan antibodi monoklonal, yang diambil dari biopsi jaringan pada hari ke 5. Rasio CD4+/CD8+didapatkan dengan membandingkan hasil penelitian CD4+ yang dilakukan peneliti lain dengan subyek dan waktu yang sama. Distribusi data variabel diuji menggunakan uji Shapiro-Wilk selanjutnya dilakukan uji beda menggunakan Mann-Whitney U Test. Untuk menilai hubungan skor histologi limfosit CD8+ dan rasio skor histologi CD4+ /CD8+ digunakan uji korelasi Spearman. Hasil : Kandungan sel CD8+ antara kelompok tanpa levobupivakain dan dengan levobupivakain berbeda bermakna (p=0,008). Rasio kandungan sel CD4+/CD8+ antara kelompok tanpa levobupivakain dan dengan levobupivakain berbeda bermakna (p= 0,009). Terdapat hubungan skor histologi limfosit CD8+ terhadap rasio CD4+/CD8+ dengan koefisien korelasi r= 0,004. Derajat kemaknaan p=- 0,817 menunjukan korelasi negatif kuat, berarti hubungan antara CD8+ dan rasio CD4+/CD8+ secara statistik bermakna. Kesimpulan : Infiltrasi anestetik lokal levobupivakain disekitar luka terbukti dapat mengurangi jumlah skor histologi CD8+ dan meningkatkan rasio CD4+/CD8+, yang mencerminkan perbaikan luka. Background : Pain control will enhance immune responses that leads to a better wound healing process. Infiltration of local anesthetic at the site of the wound is a technique of pain control. Levobupivacaine is a local anesthetic with long acting duration. Lymphocyte CD8+ plays an important role as a down-regulator of wound healing. The increased of CD4+/CD8+ ratio shows the improvement of wound healing process. In this study the profile of CD8+ and CD4+/CD8+ ratio expected to reveal the wound healing process with levobupivacaine infiltration. Objective : The aim of this study was to analyze the relation between infiltration of levobupivacaine as a local anesthetic with CD8+ histoscore and CD4+/CD8+ ratio. Method : This study was an animal study with “Randomized Post test only control group design” on ten Wistar rats. Randomly, it was divided into two groups, 5 for each. Group 1, was group with 2 cm incision at the back and injected at the site of the wound with empty syringe (without levobupivacain). Group 2, was group with 2 cm incision and infiltrated on the site of the wound with levobupivacain every 8 hours for 24 hours. Detection of CD8+ was performed with immunohistochemistry staining on the day 5th tissue biopsy and was scored by histoscore system. CD4+/CD8+ ratio was obtained from comparing histoscore of CD4+ with CD8+. The histoscore of CD4+ was obtained from another research performed by the same sample. Distribution data were analyzed by Shapiro Wilk Test and followed by Mann-Whitney U Test. The correlation between CD8+ and CD4+/CD8+ ratio were performed by Spearman correlation test. Results : This study showed that CD8+ histoscore between 2 groups was significantly different (p=0,008). CD4+/CD8+ ratio between two group was also different significantly (p=0,009). There was a significant difference in the correlation between the histoscore of CD8+ and CD4+/CD8+ ratio (r= -0,817 ; p=0,004). It revealed a negative strong correlation between the histoscore of CD8+ and CD4+/CD8+ ratio. Conclusion : Infiltration of local anesthetic at the site of the wound decreases CD8+ histoscore and increases CD4+/CD8+ ratio which will lead to an improvement of wound healing processes.
