Abstract
Latar Belakang. Reaksi kusta merupakan suatu reaksi kekebalan dengan akibat yang dapat merugikan penderita. Reaksi kusta mempunyai manifestasi klinik yang berat, meskipun penyebab dan patogenesisnya belum diketahui dengan jelas. Menurut data kusta nasional tahun 2000, sebanyak 5 % penderita kusta mengalami reaksi kusta. Penelitian ini bertujuan untuk membuktikan pengaruh faktor risiko karakteristik individu, karakteristik status klinis dan karakteristik status pengobatan terhadap terjadinya reaksi kusta. Metoda Penelitian. Penelitian ini merupakan studi kasus kontrol dengan jumlah sampel sebanyak 106 penderita. Sampel terdiri dari 53 penderita kelompok kasus dan 53 penderita kelompok kontrol, baik penderita sebelum, selama maupun sesudah pengobatan MDT tetapi masih dalam masa pengamatan. Kasus adalah semua penderita yang sudah didiagnosis mengalami reaksi kusta. Kontrol adalah penderita yang sudah didiagnosis tidak mengalami reaksi kusta. Kontrol diperoleh secara acak sederhana di tiap-tiap Puskesmas. Pengolahan data dilakukan dengan cara analisis univariat, menghitung population attributable risk (PAR), analisis bivariat dengan uji chi square dan analisis multivariat dengan uji regresi logistik ganda. Hasil Penelitian. Di Kabupaten Brebes tahun 2007 terdapat 303 penderita kusta terdaftar yang terdiri tipe PB : 25 orang dan tipe MB : 283 penderita, CDR : 1,20 / 10.000, PR : 1,73 / 10.000, cacat tingkat 2 : 4,21 %, penderita anak : 14,02 %. Hasil penelitian menunjukkan bahwa faktor risiko karakteristik status klinis yang terbukti berpengaruh terhadap terjadinya reaksi kusta adalah umur saat didiagnosis kusta lebih dari 15 tahun (OR = 4,210; p = 0,030; 95 % CI 1,150 – 15,425), lama sakit lebih dari 1 tahun (OR = 2,813; p = 0,038; 95 % CI = 1,160 – 7,464) dan kelelahan fisik (OR = 4,672; p = 0,001; 95 % CI 1,909 – 11,432). Probabilitas penderita untuk mengalami reaksi kusta dengan semua faktor risiko di atas adalah sebesar 18,8 %. Kesimpulan : Faktor risiko yang terbukti berpengaruh terhadap terjadinya reaksi kusta adalah umur saat didiagnosis kusta lebih dari 15 tahun, lama sakit lebih dari 1 tahun dan kelelahan fisik. Background. Leprosy reactions is an immune response as a consequence to suffer an injury of the leprosy patients. Leprosy reactions has a severe clinical manifestations, although such as the causes and the pathogenesis of the leprosy reactions are poorly understood. The national data of leprosy at 2000, reported that 5 % of leprosy patients had an experience of leprosy reactions. The aim of the study was to find out the risk factors of individual, clinical status and treatment status characteristic that influence to the occurrence of leprosy reactions. Methods. This study was a case control study with the number of samples 106 patients. The samples divided into 53 patients as a case group and 53 patients as a control group, occurrence of is before, during or after MDT treatment but not yet release from control. The case group was all the leprosy patients diagnosed with leprosy reactions historic experience. The control group was the leprosy patients diagnosed not with leprosy reactions historic experience. The control group patients was selected by a simple random sampling in each Puskesmas. The data were analized by univariate analysis, to count population attributable risk (PAR), bivariate analysis with chi square test and multivariate analysis with method of binary logistic regression. Results. In Brebes district at 2007 was 303 patient with leprosy divided PB : 25 patients and MB : 285 patients, case detection rate : 1,20 / 10.000, prevalensi rate : 1,73 / 10.000, grade 2 impairment : 4,21 % and children cases : 14,2 %. This reseach results showed that risk factors clinical status characteristic of leprosy reactions were age at leprosy diagnosed more than 15 years old (OR = 4,210; p = 0,030; 95 % CI = 1,150 – 15,425), duration of illness more than 1 year (OR = 2,813; p = 0,038; 95 % CI = 1,160 – 7,464) and physical fatique (OR = 4,672; p = 0,001; 95 % CI = 1,909 – 11,432). Patients probablility to have risk leprosy reactions with those all risk factors above is 18,8 %. Conclusion. Risk factors influence to the occurence of leprosy reactions is age at leprosy diagnosed more than 15 years old, duration of illness more than 1 year and physical fatique.
